POSITION DESCRIPTION

POSITION: REGISTERED DIETITIAN

RESPONSIBLE TO: Health Promotion/Disease Prevention Manager

SALARY: Step 38 ($87,240) – 50 ($124,384) 

CLASSIFICATION: Professional, Regular, Full-Time

LOCATION: Klamath Tribal Health & Family Services

Wellness Center, Chiloquin, Oregon

POSITION OBJECTIVES

This Registered Dietitian position primary responsibility is to assist with the nutritional needs of the patients seen at Klamath Tribal Health & Family Services (KTHFS). Routine duties will include providing dietary patient care and nutritional education in the clinic, at other KTHFS service location, and at alternate job sites. Other duties will include providing support services for tribal programs, nutrition program development, educating staff, and community outreach.

MAJOR DUTIES AND RESPONSIBILITIES

1. Conduct nutritional assessment and implement nutritional patient care plans consistent with current guidelines.

2. Adapt care plans with patients to be consistent with lifestyle, cultural, psychosocial, economic, and physiological factors. 

3. Document patient visits in the Electronic Health Record (EHR) in accordance with KTHFS procedures. 

4. Provide direct patient care Conduct nutritional assessment and implement nutritional patient care plans in collaboration with the multidisciplinary team.

5. Develop and maintain a KTHFS Nutrition Services Program Protocol and Procedures for common disease states treated with Medical Nutrition Therapy (MNT), dietary support of existing KTHFS programs, and community outreach events.

6. Provide MNT, develop and maintain dietary care protocols of common disease states (diabetes, chronic kidney disease, cirrhosis, hypertension, chronic heart failure, cholesterol disorders, malnutrition, malabsorption disorders, etc) that will include the guidelines followed, educational material to use, patient case-management process, scheduling process, and reporting productivity & clinical measure.

7. Develop dietary protocols for support of KTHFS and Tribal programs (medical, dental, pharmacy, and health education departments; tribal meal sites; commodities program; etc) will include the type of dietary support, educational material & guidelines used, frequency of interaction, process for other programs to request dietary support and reporting on productivity & other measure.

8. Develop dietary protocols for community outreach at KTHFS, tribal, and local events (Diabetes & Diabetes Prevention Program walks/events; Restoration; elder’s, women’s, men’s, and children’s events; education booths/set-up in KTHFS lobbies; etc) will include type/frequency of events, educational material used, the process for requesting dietitian involvement in events, and reporting on productivity & other measures.

9. Assist with food insecurity programs and operations that increase access to fresh produce including cooking demonstrations and reoccurring food pantry. 

10. Review, evaluate & select or prepare suitable nutritional education materials for patient and staff education within KTHFS departments and programs.

11. Effectively teach fundamental classes and any additional pertinent material individually, in family settings, and in a classroom environment. Ability to have flexible schedule to maximize participation will be needed. Prepare meals and/or snacks for program participants, as needed.

12. Prepare and submit reports that will include productivity and outcomes measure for direct patient care, nutritional program development, KTHFS program support, grant program support, and community outreach activities in compliance with the Privacy Act, HIPAA and KTHFS policies.

13. The incumbent will have other duties and tasks assigned within the scope of practice for a Registered Dietitian.

14. Like all employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes’ broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes’ Mission Statement. Some of these tasks may be scheduled outside of regular work hours, if necessary.

SUPERVISORY CONTROLS

Work is performed under the general direction of the Public Health Director? This position requires sound judgment, strong organizational skills, time management experience, and the ability to work independently with minimum direct supervision. 

The supervisor outlines the overall program objectives and priorities, time limit, and financial resources available. Work is reviewed by the supervisor from an overall standpoint in terms of meeting objectives, effectiveness, and compliance with laws, regulations, policies, and procedures. 

The employee plans and schedules recurring work, handles routine problems, and completes work using personal initiative. Professional knowledge and judgment must be used to the Nutrition Services Program objectives and priorities. 

KNOWLEDGE, SKILLS, ABILITIES

Knowledge of principles and practices of dietetics gained through education and experience.

Knowledge of nutrition and its relationship to health and disease in order to develop nutrition care plans and to provide training to clients and staff on a wide variety of nutrition related health topics.

Knowledge of how nutrition, physical activity and other lifestyle changes can improve disease specific health outcomes. 

Knowledge of psychosocial, cultural, and economic factors that affect individual and family nutritional/dietary care plans.

Knowledge of local community resources that affect and can improve nutritional health.

Knowledge of research, guidelines, and educational principles as applied to public health nutritional and dietary practice.

Knowledge of principles of consultation and skill in interviewing patients, analyzing dietary needs and in translating information into individual patient or family nutritional care plans.

Ability and skill to adapt consultation, training, or nutrition care plans to suit the individual patient or family.

Ability to analyze and evaluate nutrition scientific data and apply the information to the Nutrition Services Program.

Ability to prepare and distribute educational materials to staff, patients, and the community.

Ability and skill to plan, develop, and implement appropriate Nutrition Services Program policies, procedures, and protocols.

Ability to establish and maintain effective working relations with staff, community health professionals, and the public.

Ability to present ideas clearly and concisely through verbal and written communication.

Ability to plan and organize work effectively and independently.

Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping, HIPAA and Privacy Act requirements.

QUALIFICATIONS, EXPERIENCE, EDUCATION

Minimum Qualifications: Failure to comply with minimum position requirements may result in termination of employment.

· REQUIRED to have a Bachelor's or graduate/higher level degree in the fields of dietetics, food, nutrition, food service management, institution management, or related science.  (Must submit copy of diploma or transcripts with application.)

· REQUIRED Must possess and maintain Registered Dietitian or Registered Dietitian Nutritionist credentials by the Commission on Dietetic Registration (CDR).

· REQUIRED to possess an active state license as a Registered Dietitian. For out of state applicants; Oregon Licensure required within 90 days of hire. (Must submit copy of Licensure with application.)

· REQUIRED to obtain and maintain an Food Handlers Permit within 120 days of hire, (program may assist with application fee), must obtain the permit prior to any food preparation.

· REQUIRED to have experience with computer systems, electronic health records, data entry programs, and Microsoft Office

· REQUIRED to have experience with patient information confidentiality requirements. 

· REQUIRED to have a minimum of 2 years experience providing direct dietary patient care (MNT) and interacting with providers.

· REQUIRED to submit to annual TB skin testing and adhere to KTHFS staff immunization policy in accordance with the Centers for Disease Control immunization recommendations for healthcare workers. 

· REQUIRED to accept the responsibility of a mandatory reporter of abuse and neglect of infants and children, people who are elderly or dependent, individuals with mental illness or developmental disabilities or residents of nursing homes and other health care facilities. This includes reporting any evidence of physical injury, neglect, sexual or emotional abuse or financial exploitation.

· REQUIRED to acquire and maintain BLS certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification.

· REQUIRED to submit to a background and character investigation, as per Tribal policy. Following hire must immediately report to Human Resource any citation, arrest, conviction for a misdemeanor or felony crime.

Preferred Qualifications:

· A graduate degree in Nutrition or Public Health is preferred.

· Four years experience providing outpatient dietary care (MNT), interacting with interdisciplinary team and community nutrition is preferred.

· A specialized certification such as Certified Diabetes Care & Education Specialist (CDCES) or Certified Specialist Obesity & Weight Management (CSOWM) is preferred

· Experience working with Native Americans is preferred. 

Indian Preference:

· Indian and Tribal Preference will apply, as per policy. (Must submit tribal documentation with application to qualify for Indian Preference).
