Job Type: Full-time, Benefit-eligible Position

Salary: $57,000 - 70,000 (Exact compensation may vary based on skills and experience)

Sign-On Bonus: $1000 Sign on Longevity bonus (with $500 paid out after 6 months and the rest paid out after 12months)

Location: Newton, MA

Supervisor: P2P Deputy Director

Schedule: 40 Hours per week/Full Time (Monday - Friday, 10am-6pm)


PURPOSE:

Pathway to Possible is a congregate care facility and allows independent living and assisted living for adults with developmental disabilities in the Newton and Wellesley areas. The program allows for housing for adults referred to us by the Department of Developmental Services and to increase their self-sufficiency and independent living skill development. The Program support adults to:

  • Develop skills and abilities needed to be productive members of their residential program.
  • Develop skills and abilities needed to live independently if able.
  • Establish and promote lifelong connections.

Pathway to Possible is responsible for providing a safe and healthy environment, and welcoming atmosphere, while ensuring the safety of the residents. This position is responsible for reminding residents of their daily schedules & providing supportive companionship. Our staff are expected to be strong role models for the clients we serve and have the ability to develop positive relationships with our clients.


JOB DESCRIPTION:

The Program Manager is responsible for the overall operation of the house, ensuring high quality services to individuals, scheduling and supervising all support staff, fiduciary oversight, overseeing med distribution and administrative duties as required by P2P, DDS and DPH.


RESPONSIBILITIES:

  • Follow all policies and procedures as outlined in the Pathway to Possible (P2P) Personnel Policies and Procedures and the P2P Policies to Promote Human Dignity.
  • Provide regular documented supervision and feedback to direct service staff ensuring the safety and well-being of all clients.
  • Schedule and approve staffing to provide 24/7 coverage at the home.
  • Provide up to 10 weeks of on-call coverage per year.
  • Be an active member of a management team, participating in agency-wide events, staff meetings and training.
  • Hold monthly staff meetings and conduct annual staff evaluations.
  • Ensure that all consumers' health care and dietary needs are being met and medication administration regulations are consistently applied and complied with.
  • Maintain a thorough knowledge of consumers' financial resources, including government benefits, housing subsidies and other resources that they may be entitled to. Ensure client compliance with entitlement benefits.
  • Ensure that all safety programs and policies are communicated to staff and are implemented and practiced consistently, including ensuring all safety plans are accurate and current.
  • Ensure ISP plans are developed and implemented in accordance with P2P and DDS guidelines. Ensure that the proper monthly documentation is completed.
  • Direct case managers to oversee the monitoring and scheduling of medical/dental appointments for each resident in the home.
  • Participate in P2P and Department of Developmental Services training programs and attend all Program Manager meetings.
  • Monitor and coordinate with HMO to ensure proper operation of house, grounds, vehicles, furnishings and equipment, and physical plant.
  • Maintain all program records according to established guidelines and state and federal requirements.
  • Maintain financial accounts and budgetary guidelines in alignment with state, federal and P2P guidelines.
  • Maintain relationships with involved families, guardians, and other providers.

Requirements

QUALIFICATIONS/REQUIREMENTS:

  • Bachelor's degree in human services or related field is preferred
  • A graduate degree and/or 5 years program management experience is preferred.
  • A commitment to the empowerment of individuals with intellectual and developmental disabilities
  • Familiarity with the DDS delivery system
  • First AID and CPR Certified and Experience with individuals who have complicated medical needs is preferred.
  • Strong written and verbal communication skills to be used across the P2P Community.

Physical Requirements:

  • Must be able to lift up to 15 pounds
  • Must be able to provide physical assistance to adults using wheelchairs
  • Must be able to evacuate individuals supported within 2 minutes
  • Must be able to assist individuals in transferring from sitting to standing

Other requirements:

  • Proficient with Microsoft Office Suite or related software.
  • Legally authorized to work in the United States.
  • Valid Massachusetts driver's license.
  • Clearance of CORI and SORI background checks as required by state and federal law.
  • Clearance of DPPC registry as required by state law
  • A cleared search of finger printing through the DDS criminal background check
  • Current MAP certification and maintain MAP certification for the duration of employment.
  • Comfortable working in an environment with a therapeutic cat.
