Job description

he Rural Health Transformation Program Manager is responsible for leading the implementation, coordination, and successful execution of large-scale strategic initiatives funded through the Rural Health Transformation Program and other organizational investments.

Working closely with the CEO, senior leadership, program managers, finance, clinical leaders, and external partners, this position ensures projects are delivered on time, within budget, and in compliance with grant requirements while achieving intended outcomes.

The Program Manager serves as the central point of coordination across multiple departments, translating strategic priorities into executable work plans, monitoring progress, identifying barriers, and ensuring accountability throughout implementation.

Over time, this position can assume increasing responsibility for organizational grants management, program development, and strategic funding initiatives, serving as a succession pathway into the Manager of Grants & Program Development role.

Primary Responsibilities:

· Lead the implementation of Rural Health Transformation Program (RHTP) initiatives and other CEO-designated strategic initiatives including, but not limited to, enhanced chronic disease management, technology upgrades, and other funded initiatives.

· Coordinate development of detailed project work plans, implementation timelines, budgets, and project documentation based on the applications submitted.

· Coordinate cross-functional project teams to ensure timely completion of project milestones and deliverables.

· Monitor project performance and recommend corrective actions when necessary.

· Facilitate project meetings, maintain action plans, and ensure accountability among project stakeholders.

· Track project budgets and collaborate with Finance to monitor grant expenditures and reimbursement requirements.

· Coordinate implementation activities required under federal, state, and private grant awards.

· Work collaboratively with the Manager of Grants & Program Development to ensure grant objectives, deliverables, reporting requirements, and compliance obligations are met.

· Assist with preparation of grant reports by collecting outcome data, coordinating narrative updates, and maintaining supporting documentation.

· Develop and maintain project tracking tools, dashboards, and executive reports for senior leadership and the Board of Directors.

· Collaborate with department leaders to implement new programs, workflows, and operational improvements resulting from grant-funded initiatives.

· Serve as the primary liaison between internal departments, consultants, vendors, community partners, and funding agencies for assigned initiatives.

· Support organizational change management efforts by facilitating communication, training coordination, and stakeholder engagement.

· Analyze project outcomes and recommend process improvements to increase efficiency, sustainability, and organizational effectiveness.

· Assist in identifying future funding opportunities and participate in program planning and development activities.

· Assume increasing responsibility for grants management, program development, and strategic initiatives as organizational needs evolve.

Requirements

Qualifications:

· Minimum of a bachelor's degree in Healthcare Administration, Public Administration, Business Administration, Public Health, Nonprofit Management, or a related field.

· Minimum of three years of experience managing complex projects, strategic initiatives, grants, healthcare operations, or related work experience.

· Experience coordinating multidisciplinary teams and managing multiple priorities simultaneously.

· Experience working within a healthcare, nonprofit, governmental, or community-based organization preferred.

· Experience managing federal, state, or foundation grant-funded programs preferred.

· Experience with Federally Qualified Health Centers (FQHCs), HRSA programs, or value-based care initiatives preferred.

Knowledge, Skills, and Abilities:

· Exceptional organizational skills with the ability to manage multiple complex initiatives simultaneously.

· Strong project management skills, including planning, implementation, monitoring, and evaluation.

· Ability to organize large amounts of information and ensure project deadlines, grant requirements, and reporting timelines are consistently met.

· Excellent written and verbal communication skills.

· Strong facilitation skills with the ability to coordinate cross-functional teams and build consensus among diverse stakeholders.

· Ability to develop project plans, implementation schedules, dashboards, and executive reports.

· Strong analytical and problem-solving skills with the ability to identify risks and implement effective solutions.

· Knowledge of grant administration, grant compliance, and reporting requirements.

· Ability to interpret grant requirements and translate them into operational work plans.

· Ability to build collaborative relationships with internal staff, community partners, consultants, funding agencies, and external stakeholders.

· Ability to manage organizational change and support implementation of new programs and workflows.

· Knowledge of quality improvement methodologies, healthcare operations, and performance measurement preferred.

· Ability to analyze data, evaluate program outcomes, and communicate results using meaningful performance measures.

· Proficiency with Microsoft Office applications and project management software.

· Ability to exercise sound judgment, maintain confidentiality, and work independently while supporting organizational priorities.
