Program Coordinator

AmeriServe International provides supported community living and day habilitation services for individuals with intellectual disabilities. As a Program Coordinator, you will supervise Client Service Coordinators (CSCs) and team members who support individuals with intellectual disabilities. Your leadership ensures services are delivered with quality, consistency, and compassion. While direct support may sometimes be required due to staffing or scheduled PTO for team members, your primary focus is guiding staff, maintaining state and Ameriserve compliance, and fostering independence and community integration for those we serve. Key Responsibilities:

Oversee implementation of person-centered support plans (PCSPs) by directing interdisciplinary teams and ensuring quality outcomes.

Provide managerial oversight of InterRai processes to ensure proper utilization and compliance.

Ensure service eligibility is established and maintained.

Oversee and quality-assure documentation and storage of client information related to eligibility, program development, implementation, and habilitation planning.

Manage development and monitoring of service and transition plans, ensuring alignment with organizational standards.

Provide leadership in the development, review, and revision of habilitation plans, ensuring consistency with individual needs and company goals.

Develop and supervise progress reporting and ensure alignment with company standards.

Train team members on habilitation plans, providing continued monitoring of outcomes.

Direct emergency preparedness training for staff and ensure systems support safe and independent living.

Coach team members and individuals served in the development of daily living skills, including:

Building community relationships

Engaging in personal interests and community activities

Mobility and transportation training

Use of adaptive aids and equipment

Complete daily, weekly, and monthly reports.

Document services to demonstrate progress toward goals.

Participate in planning, coordinating, and facilitating employee training programs, including New Employee Certification and Continuing Education.

Hire, train, schedule, and supervise team members.

Conduct audits, manage program budgets, and monitor financial performance.

Participate in on-call rotation and contribute to service expansion initiatives.

Other duties as assigned by the supervisor.
