Job description

We are currently seeking a Social Service Worker.

Job Responsibilities and Duties:
    • Plan, develop, organize, implement, evaluate, and direct the overall operations of the Social Services Department
    • Oversee the Medicaid process
    • Complete MDS assessments and support the admissions process
    • Provide leadership and guidance to the Social Services Department staff
    • Work primarily during daytime hours, with occasional evening and weekend visits as needed
Requirements:
  • Bachelor’s degree from an accredited school or program of Social Work (minimum requirement)
  • Two years of postgraduate experience in long-term care or related programs
  • Must meet equivalent state requirements for social work practice, or hold certification/credentialing from the National Association of Social Workers if the state has no legal regulation
  • BS in Social Service required
  • 2+ years of social work and MDS assessment experience, ideally in a long-term care setting
  • Energetic, results-oriented team leader with superior interpersonal and communication skills
  • Self-motivated, caring approach, with passion for working with elderly and behavioral residents
