This PRN position offers ultimate flexibility-you tell us when you are available, making it the perfect role to supplement your current income while providing critical, one-on-one care to a vulnerable patient population.DutiesClient Assessments: Conduct comprehensive in-home initial assessments, routine reassessments, and recertifications for private pay and Medicaid waiver patients.Care Planning: Develop, implement, and update individualized plans of care based on the patient's physical, emotional, and environmental needs.Territory Navigation: Drive efficiently across Lake, Lorain, Cuyahoga, Medina, and Geauga counties to reach patients in their homes.Clinical Documentation: Maintain accurate, timely, and fully compliant electronic medical records (EMR) for all assessments in accordance with state and Medicaid guidelines.Care Coordination: Collaborate seamlessly with internal clinical supervisors, physicians, and care team members to ensure continuity of care.What We Are Looking For:Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Ohio.
