• Conduct thorough assessments and develop personalized treatment plans for clients, incorporating crisis prevention and management strategies.
• Provide timely and accurate documentation in compliance with regulatory standards and agency policies.
• Utilize evidence-based screening tools to gather data and inform treatment decisions in collaboration with the clinical team.
• Work in partnership with interdisciplinary teams to design and implement positive behavior support plans tailored to individual client needs.
• Take an active role in all licensing and accreditation processes to ensure best practices are upheld at all times.
• Act as a liaison between clients and relevant service systems, including state agencies and community organizations, to provide comprehensive support.
• Maintain effective communication channels to meet contract standards and align with the mission of Prime Health Counseling within the community.
• Develop and nurture relationships with local, state, and national agencies to expand program visibility and secure new partnerships and funding.
• Engage in professional development activities, including attending conferences and representing the agency in various forums.
• Promote and integrate the agency's mission, values, and philosophy into everyday practice.
• Perform additional tasks as assigned by the Clinical Director, ensuring alignment with organizational goals.

You will work with a diverse patient population that includes individuals seeking assistance with emotional health challenges, substance use disorders, developmental disabilities, and those recovering from brain injuries. • A Master’s Degree in a mental health-related field is required to ensure a strong foundation of knowledge and skills.
• An independent LMHC license is mandatory for this role, validating your expertise and ability to provide high-quality counseling.

• Experience working within community health settings or knowledge of mental health service delivery systems is beneficial.
• Familiarity with evidence-based practices and screening tools that aid in treatment planning is highly regarded.
• Previous experience collaborating with interdisciplinary teams and community partners to support client needs.
