Prime Health Counseling, LLC is actively seeking a dedicated and compassionate Licensed Mental Health Counselor (LMHC) to join our dynamic team in Brockton, Massachusetts. Key Responsibilities
  • Conduct thorough assessments and develop personalized treatment plans for clients, incorporating crisis prevention and management strategies.
  • Provide timely and accurate documentation in compliance with regulatory standards and agency policies.
  • Utilize evidence-based screening tools to gather data and inform treatment decisions in collaboration with the clinical team.
  • Work in partnership with interdisciplinary teams to design and implement positive behavior support plans tailored to individual client needs.
  • Take an active role in all licensing and accreditation processes to ensure best practices are upheld at all times.
  • Act as a liaison between clients and relevant service systems, including state agencies and community organizations, to provide comprehensive support.
  • Maintain effective communication channels to meet contract standards and align with the mission of Prime Health Counseling within the community.
  • Develop and nurture relationships with local, state, and national agencies to expand program visibility and secure new partnerships and funding.
  • Engage in professional development activities, including attending conferences and representing the agency in various forums.
  • Promote and integrate the agency's mission, values, and philosophy into everyday practice.
  • Perform additional tasks as assigned by the Clinical Director, ensuring alignment with organizational goals.
Patient Population / Client Focus

Qualifications
