Conviva Senior Primary Care, a subsidiary of Humana Inc., is recruiting a full-time primary care physician for our Ormond Beach, Florida center. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director.

The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy.

A day in the life of a Conviva Primary Care Physician:

  • Monday-Friday 8:00am-5:00pm
  • Join an experienced team with an established patient panel
  • Opportunity to spend more time with patients
  • Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care
  • Minimal, shared call rotation

Use your skills to make an impact

Required Qualifications

  • Doctor of Medicine (MD) or Osteopathic (DO)
  • Board certified or board eligible in Family Medicine or Internal Medicine - Geriatrics specialty would be ideal
  • Valid, unrestricted license (or willing to obtain) in the state of desired employment (Florida)
  • Current, unrestricted DEA license

Preferred Qualifications:

  • Minimum of two to five years directly applicable experience preferred
  • New graduates will be considered
  • Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
  • Knowledge of Medicare guidelines and coverage
  • Knowledge of HEDIS quality indicators
  • Bilingual (English and Spanish) is a plus

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


$255,000 - $298,500 per year


This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
