We provide primary care and other services to seniors and families in our medical centers that are comfortable, convenient and staffed with caring professionals. Our activity and wellness facilities are filled with health plan members who know us for our distinct events and programs that cater to the communities we serve.

Responsibilities
  • Provide comprehensive primary care services to adult and senior population within the Medicare Advantage/Medicare/Medicaid model.
  • Conduct routine and preventative visits, including annual wellness exams, chronic disease management, and acute care visits.
  • Order outside diagnostic tests and lab work as needed (blood/urine test, tissue samples, imagery, etc.).
  • Develop and implement personalized care plans focused on improving patient outcomes, quality and satisfaction.
  • Manage patients using a value‑based approach, emphasizing prevention, coordinated care, and reduction of avoidable hospitalization.
  • Collaborate closely with care teams including nurse practitioners, medical assistants, case managers and specialists to insure seamless patient care.
  • Review and address quality metrics, including HEDIS and STAR measures, to optimize clinical performance and patient outcomes.
  • Accurately and on a timely basis document encounters and diagnoses in eCW, ensuring complete and compliant coding for risk adjustment.
  • Participate in multidisciplinary meetings and quality improvement initiatives aimed at enhancing clinical outcomes and patient satisfaction.
  • Maintain ongoing communication with patients and their families, providing education on disease prevention, medication management and health lifestyle choices.
  • Adhere to Palm Primary Care's protocols, standards of care, and compliance with all regulatory requirements.
  • Maintain confidentiality and always work with respect and integrity.
  • Promote a positive, team‑oriented culture consistent with Palm Primary Care's mission to deliver exceptional, patient‑focused care.
Qualifications
  • Medical Degree (M.D., D.O.) from an accredited institution.
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine.
  • Active, unrestricted Texas Medical License.
  • Current DEA license in good standing.
  • Minimum 2‑3 years’ experience providing care to Medicare and/or Medicare Advantage patients preferred.
  • Experience working in a value‑based care model, focused on outcomes, quality metrics, and patient satisfaction.
  • Strong understanding of HEDIS, STAR ratings, risk adjustment, and preventative care measures.
  • Demonstrated ability to manage chronic conditions and promote population health in a senior patient population.
  • Excellent communication, collaboration, and interpersonal skills for a multidisciplinary team environment.
  • Comfortable using EMR systems (eCW a plus).
  • Commitment to providing high‑quality, patient‑centered care aligned with Palm Primary Care's mission and standards.
