Caroleton Healthcare Center, a member of the CommuniCare Family of Companies is currently recruiting for a Transportation Assistant to join our team.

Part Time Position Available

Shift: 1st /2nd shift

Rate: $18/hour

PURPOSE/BELIEF STATEMENT:

The position of Transportation Assistant exists to assist residents with transportation to and from inside facility and outside physician appointments and events in a safe manner and in accordance with established procedures. This position must work effectively with team members in ensuring that work is accomplished and quality care is delivered, supporting their team members and celebrating team successes. While focusing on delivering quality resident care, the position must also manage the resources that are available to them in their job.

JOB DUTIES & RESPONSIBILITIES

  • This will be a fluctuating scheduling based off of resident and facility needs. Hours vary between 15-29 hours a week. Most Transportation needs are between 6 am to 6 pm. Transportation needs are both local and non-local. In this position you will utilize the facility 15 passenger mini-bus.

  • Transport and assist residents, per facility vehicle, to outside physician visits in a safe manner.

  • Assist with transporting residents within the facility to scheduled specialty appointments, i.e. podiatry, audiology, optometry, dental, etc.

  • Assist with transporting residents to and from scheduled in-house activities.

  • Provide one on one monitoring for residents deemed an elopement risk on outside visits.

  • Perform basic nursing care procedures for residents as required.

  • Create a clean, comfortable, and safe environment for residents and assure that their needs are met.

  • Maintain resident documentation records.

  • Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.

  • Attend and participate in scheduled training, educational classes, and orientation programs to maintain certification and enhance quality of care.

  • Perform other related activities as assigned or requested.

QUALIFICATIONS & EXPERIENCE REQUIREMENTS

  • Must be at least 21 years of age. Must have a valid active driver's license. Must have a clean driving record. No Citations or accidents in the Past 10 years. Public Transportation experience preferred but not required. Part Time Transportation must be available to work Weekends.

  • High school graduate or G. E. D. equivalent

  • Must possess a current, valid Driver's License in State of employment.

  • 2+ years driving experience

  • Candidate must have a clean driving record.

  • Graduate of an approved Nurse Aide Training Program or be eligible to enter such a program and graduate and pass a state test within 120 days from date of employment.

  • Must possess Red Cross First Aid Certificate.

  • Prior work/life experiences involving independent decision making

  • Prior work/life experiences, preferably in healthcare or a service industry

  • Compassionate and enthusiastic personality
