General Summary:

Under limited supervision, serves as a team lead for the community violence prevention and intimate partner violence (IPV) prevention specialists. Responsible for case management services to enrolled population, represents the program in the hospital and the community, advocates and offers supportive services across many systems to individuals who have been victims of violence/ violent crime. Additionally, the team lead strengthens the program model by participating in community events, hospital and state-wide task forces, research components of the intervention, youth violence prevention, and other supportive aspects of service delivery. The team lead will be responsible for data analysis and programmatic evaluation of the Violence Prevention Program to identify areas of need and program improvement.

Principal Responsibilities and Tasks:

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

1. Lead a team of dedicated professionals, providing guidance, mentorship, and support to ensure their professional growth and success.
2. Foster a positive and inclusive team culture that encourages collaboration, innovation, and a shared commitment to the program's mission.
3. Conduct regular team meetings and skill-building activities to enhance team effectiveness.
4. Conduct performance evaluations for Violence Prevention Specialist staff.
5. Create and maintain staffing schedule for Violence Prevention team to ensure appropriate 7 day/week coverage.
6. Conducts brief psychosocial, needs assessment, and mental health screenings to identify potential service needs for eligible patients.
7. Maintains an individual caseload of 10 clients. Interfaces with client, in the hospital and the community, on a regular basis to help him/her practice new skill sets while he/she pursues community referrals and personal goals. Develops individual action plans; serves as a role model for the client, acts as an advocate and accompany clients in court settings, and provides crisis intervention and/or conflict mediation when needed ; preparation of documents for relevant needs.
8. Provides supportive counseling and guidance, uses mental health first aid and other crisis intervention techniques to deescalate client in times of crisis and to encourage compliance with mental health services
9. Co-facilitates weekly peer support group, brings clients together, talks to participants about his/her own life experiences re: judgement and risk taking behavior, etc.; designs and markets support group and analyzes support group outcomes/data for process improvement.
10. Supports and facilitates the implementation and development of Violence Intervention Program service components according to program protocols, quality control requirements, and best practice standards.
11. Represents the Violence Intervention Program at community meetings and on committees at both the organizational and community level.
12. Participates in team discussions and communications regularly with staff; work closely with the rest of the multidisciplinary team to ensure that all program participants are receiving the services best suited to meet their needs.
13. Keeps timely documentation and submits statistical reports for review within given timelines
14. Leads weekly team meetings with Violence Intervention Team staff including facilitating discussions of caseloads, current community concerns, areas of need for the program.
15. Collects and manages baseline and follow up data from Violence Intervention Program (VIP) participants. Enters and updates data. Analyzes data from VIP program for program improvement. 
16. Helps maintain a community resource database through establishing relationships in the community, making referrals, following leads, and engaging in reciprocating services where overlap exists
17. Participates in planning and implementing annual events
18. Supports primary prevention components of the Violence Prevention Participates in community events as a representative of the Violence Intervention Program such as Youth Violence Prevention activities and collaborative projects

1.  Effective verbal, written communication, interpersonal and listening skills are required.
2. Ability to establish and maintain effective working relationships and work as part of a multidisciplinary team is required.
3. Ability to apply various interviewing techniques with diverse groups of people is required.
4. Ability to network with and provide information to key groups, individuals, and stakeholders is required.
5. Ability to handle sensitive and confidential matters with discretion and tact is required. The ability to maintain poise and professional courtesy under pressure is required. 
6. Writing and typing skills required to keep accurate case documentation.
7. Strong organizational skills required.
8. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population - specific and age - specific needs. 

Education and Experience:

1. Bachelor's Degree in sociology, psychology, social work or related field required.
2. Two (2) years in leadership or supervisory role required
3. Five (5) years in community outreach, health case management, community violence intervention, social services, or direct client services required
4. Master's Degree Preferred
5. LCSW-C preferred
6. Background in substance abuse, traumatic stress, motivational interviewing, and/ or violence prevention/ intervention preferred.
