Prevention Point Philadelphia (PPP) is a nationally recognized harm reduction and public health organization dedicated to promoting health, dignity, and stability for communities impacted by substance use, homelessness, poverty, and systemic inequity. Founded in 1991 during the height of the HIV/AIDS crisis, PPP has grown from a grassroots syringe exchange program into a comprehensive, low-barrier health and social service organization serving thousands of Philadelphians each year.
HARM REDUCTION MEDICAL SERVICES OVERVIEW:
Grounded in harm reduction, PPP meets people where they are with compassion, respect, and nonjudgmental care. PPP provides integrated medical and behavioral health services including primary care, wound care, medication for opioid use disorder, HIV and hepatitis C testing and treatment, overdose prevention, naloxone distribution, syringe services, case management, housing and homeless outreach support, and mobile medical outreach. Services are designed to reduce harm, increase safety, improve health outcomes, and support each personโs autonomy and self-defined goals.

POSITION SUMMARY:
The Harm Reduction Sexual Health Nurse Practitioner will provide a range of integrated medical care at PPP, including treatment for substance use as well as preventative care and treatment for infectious diseases. Direct provision of care will include: STI testing, treatment, and counseling, provide pre-exposure prophylaxisย PrEP) counseling, initiation and follow up (injectable and oral), post-exposure prophylaxis (PEP) counseling, initiation and follow up, screening and treatment of Hepatitis C, treatment of HIV for a small subset of participants, treatment of basic wounds, particularly those related to substance use, and assessment of reproductive health needs and either initiation of LARC as requested, administration of long acting medications for opioid use, MAT medication adherence counseling and management,

SPECIFIC DUTIES AND RESPONSIBILITIES:
ยทย ย ย ย ย Assistance with the integration of PPPโs Medication Assisted Treatment and Recovery Clinic with Hepatitis C, HIV, PrEP, PEP, STI treatment, Wound Care, and Reproductive Health services at PPP with direction from the HIV providers, Medical Director, and Director of HIV Services and actively collaborate with other medical staff and contracted medical clinicians, as well as non-clinical program Coordinators and Navigators/Case Managers to provide high quality integrated medical care and care coordination to improve participant health and quality of life
ยทย ย ย ย ย Provide care for PPP patients including primary care, substance use treatment, PrEP, PEP, HCV treatment, STI treatment, Reproductive Health services, and other appropriate forms of care.
ยทย ย ย ย ย Review Medication Assisted Treatment and Recovery clinic participants in panel for necessary lab work, and/or referral and treatment for hepatitis C, HIV, PrEP, PEP, STI treatment, Wound Care, and Reproductive Health services
ยทย ย ย ย ย Provide clinical specialty care including Hepatitis C, PrEP, PEP, HIV, STI treatment, Wound Care, and Reproductive Health care for participants in PPPโs Medication Assisted Treatment and Recovery Clinic
ยทย ย ย ย ย Develop treatment care plans that holistically address substance use, hepatitis, HIV or prevention of HIV, wounds, and reproductive health care needs
ยทย ย ย ย ย Conduct initial prescribing as well as ongoing medication management of individuals receiving substance use treatment services on site, including prescription of MOUD, as well as prescription of HIV, HCV, PrEP, STI treatment and PEP medications as appropriate based on testing results and interest in care
ยทย ย ย ย ย Conduct in depth MAT and other medication adherence counseling in coordination with the Medical Director and all practice specialty clinicians, including novel strategies related to medication adherence which may involve transition to long acting medications, bridge and co-prescription of initial psychotropic medications for untreated mental health conditions, lower barrier MAT treatment for individuals with longer chronic substance use histories, on-site storage and administration of medication for unsheltered individuals, directly observed therapy, and community based street side medicine, outreach, and medication support
ยทย ย ย ย ย Provide health counseling and education to primary care participants and specialty populations including people living with HIV/HCV, people who use drugs, those experiencing homelessness, and gender diverse persons
ยทย ย ย ย ย Identify, evaluate, and address disease prevention and health promotion issues for clinic participants on topics such as safer drug use, wound management, prevention of HIV/HCV transmission, and sex worker safety
ยทย ย ย ย ย Document patient encounters and health information in compliance with state regulatory standards and clinic policies and procedures in PPPโs EMR within 6-12 hours of provision of care
ยทย ย ย ย ย Assist in implementing protocols and procedures to facilitate delivery of comprehensive health services
ยทย ย ย ย ย Participate in-service training for all staff in selected topics of personal specialty as deemed necessary
ยทย ย ย ย ย Perform additional duties as requested supervisors or by the leadership team

REQUIRED QUALIFICATIONSย
ยทย ย ย ย ย ย ย ย Current Certified Registered Nurse Practitioner license from the Commonwealth of Pennsylvania
ยทย ย ย ย ย ย ย ย At least 1 year experience working in an ambulatory care setting
ยทย ย ย ย ย ย ย ย Experience providing PrEP, PEP, STI treatment, sexual health services to people who use drugs preferred
ยทย ย ย ย ย ย ย ย Commitment to social justice, harm reduction and trauma-informed care
ยทย ย ย ย ย ย ย ย Strong interpersonal skills and a collaborative work approach
ยทย ย ย ย ย ย ย ย Strong bed-side manner and communication abilities
ยทย ย ย ย ย ย ย ย Ability to work well and communicate effectively with people of varying educational, socioeconomic, and cultural backgrounds.
