Position Summary:  The Medical Assistant will play a vital role in providing support to a team of nurses and nurse practitioners in delivering exceptional patient care. The role involves monitoring patient vital signs remotely through dashboards, accurately documenting patient information, assisting with patient inquiries, managing medical records, and ensuring compliance with the RPM program and healthcare regulations.


Key Responsibilities: 

  • Collaborate with Nurse Practitioners, Care Navigators, and administrative staff to monitor patient health metrics, ensure accurate data entry, and coordinate follow-up care. Act as a liaison between patients and care teams to ensure continuity of care across RPM and CCM services.
  • Address alerts on the dashboard and promptly communicate with patients as needed.
  • Demonstrate exceptional phone communication skills, conveying empathy and professionalism in all patient interactions.
  • Remind patients to take required measurements and accurately document the results in patient charts.
  • Escalate patient calls to nurses or nurse practitioners when necessary for further assessment and care.
  • Maintain and update patient medical history, vital statistics, and test results within the Electronic Medical Record (EMR) system.
  • Manage incoming and outgoing calls to patients, addressing inquiries, monitoring vital signs through RPM dashboards, and escalating cases to Nurse Practitioners when necessary. Ensure all patient interactions are documented in the EMR system and follow up with patients to encourage adherence to their care plans.
  • Perform data entry tasks in Salesforce, including closing opportunities and monitoring opportunities in the system.
  • Conduct outbound calls to patients, providing instructions, gathering information, and addressing concerns.
  • Use EMR systems, specifically Advanced MD, and CCIQ to input and manage patient data and information.
  • Verify patient insurance coverage and obtain prior authorizations as required.
  • Schedule virtual patient appointments with Nurse Practitioners, coordinating with patients and the healthcare team.
  • Ensure strict adherence to healthcare regulations, including HIPAA, and uphold the organization's code of conduct.


Required Qualifications:

  • High School Diploma or GED.
  • 1-2 years of relevant experience in a healthcare setting or medical practice.
  • Strong grasp of patient care procedures and medical terminology.
  • Excellent interpersonal skills to effectively interact with patients and healthcare professionals.
  • Strong organizational skills to manage patient records and appointment scheduling.
  • Exceptional customer service skills to provide empathetic and supportive patient interactions.


Preferred Qualifications:

  • College Degree a huge plus.
  • Bilingual in Chinese,  Spanish, Creole, or Hindi is preferred.
  • Medical Assistant Certification is highly preferred.
  • Proficiency in using EMR systems, experience in AdvancedMD a plus.
