Position Title: Community Outreach Manager

Location:Florida (Tampa, Lutz, Lakeland, Odessa)

Position Summary:As a POD Health Community Outreach Manager, you will be responsible for driving growth and expansion of POD Healths services through provider-based and community referral development. This role focuses on building strong relationships with medical offices, senior-focused organizations, and community partners to generate Start of Care (SOC) opportunities for POD Health.You will serve as a key ambassador for POD Health educating partners on our services, identifying eligible patients, and ensuring a smooth referral-to-enrollment process in collaboration with internal teams.

Key Responsibilities:

  • Proactively identify and cultivate Medicare-eligible referral sources to grow POD Healths patient census.
  • Build and maintain relationships with, but not limited to: Primary care practices, Specialist offices, Community health organizations, Senior centers, and advocacy groups
  • Educate referral partners on POD Health services, eligibility requirements, and patient benefits.
  • Drive POD Start of Care (SOC) volume by consistently generating and following up on qualified referrals.
  • Plan, coordinate, and host community and provider-focused events, including:Health education workshops,Enrollment events,Senior center presentations,Provider lunch-and-learns
  • Represent POD Health at community events, health fairs, provider meetings, and networking opportunities.
  • Collaborate closely with intake, clinical, and operations teams to ensure timely follow-up and successful SOC conversions.
  • Track and document all outreach activity, referrals, and follow-ups accurately in Salesforce within required timelines.
  • Support POD Health growth initiatives, pilot programs, and special outreach campaigns as assigned.
  • Maintain compliance with Medicare marketing guidelines and organizational standards.


Key Performance Indicators (KPIs)

  • POD Start of Care (SOC) Production
  • Referral Growth: Consistent generation of qualified Medicare referrals month-over-month
  • Provider & Community Outreach: Engage with new providers or community referral partners per month
  • Strategic Partnerships: Establish new Medicare-focused referral partnerships per quarter
  • CRM Accuracy & Timeliness: 100% accurate Salesforce documentation of outreach or referral activity
  • Conversion Rate: Maintain a referral-to-SOC conversion


Required Qualifications:

  • Bilingual Spanish fluency required
  • 3+ years of experience in:Healthcare marketing,Provider outreach,Community-based business development,Medicare or value-based care preferred
  • Strong understanding of Medicare populations and community healthcare dynamics
  • Proven ability to build provider relationships and convert referrals into enrolled patients
  • Excellent interpersonal, communication, and presentation skills
  • Cultural competency and experience working with diverse communities
  • Proficiency in Microsoft Office and Salesforce CRM (or comparable systems)
  • Valid drivers license and reliable transportation for local travel
