About Us:
At Riverside Post Acute Care, we are committed to delivering compassionate, high-quality care to our residents. We are currently seeking a detail-oriented and dependable MDS Assistant to support our MDS Coordinator in the assessment and documentation process. This is a great opportunity for someone looking to grow within the field of long-term care and gain experience in the Minimum Data Set (MDS) process.

Job Summary:
The MDS Assistant will support the MDS Coordinator in collecting, verifying, and entering accurate resident assessment data in compliance with state and federal regulations. This role is vital in helping ensure accurate reimbursement and supporting quality care outcomes.

Key Responsibilities:
  • Assist with scheduling and tracking MDS assessments
  • Gather data and documentation from clinical staff to support timely MDS completion
  • Enter MDS data into electronic health records and verify for accuracy
  • Support preparation and submission of MDS assessments to CMS
  • Maintain assessment calendars and assist with ensuring timely completion of all required assessments
  • Assist with Quality Measures, CAAs, and care planning as assigned
  • Communicate effectively with interdisciplinary team members regarding resident changes and documentation needs
  • Support compliance with PDPM and reimbursement regulations
  • Perform clerical duties such as filing, scanning, and managing resident documentation as needed

Qualifications:
  • High school diploma or equivalent required
  • Prior experience in long-term care, healthcare documentation, or MDS preferred
  • Familiarity with EMR systems (PointClickCare preferred)
  • Basic knowledge of MDS, PDPM, and regulatory requirements is a plus
  • Strong organizational and communication skills
