As a full-time Physical Therapist at Pinnacle Rehab Associates, you will work onsite within clients' homes, facilitating a comfortable and familiar environment for rehabilitation. This setting allows you to conduct evaluations, implement treatment plans, and adapt to each client's needs in real time. You will collaborate closely with interdisciplinary teams, including physicians, nurses, and social workers, all dedicated to enhancing the patient experience.

Your responsibilities will extend beyond standard physical therapy duties, reflecting the unique challenges of home care. Key tasks will include:

• Directing physical therapy treatment plans aimed at preventing disability and relieving pain while restoring maximum possible function.
• Overseeing Physical Therapy Assistants to ensure effective delivery of therapy as per established plans of care.
• Conducting thorough evaluations using diagnostic tests to assess muscle, nerve, joint, and functional capabilities.
• Analyzing the home environment for rehabilitation needs and providing recommendations to promote client independence.
• Administering prescribed physical therapy services, including manual therapeutic exercises, traction, and applications of physical agents.
• Collaborating with interdisciplinary teams to contribute to case conferences, treatment planning, and evaluation of treatment efficacy.
• Documenting all clinical records including evaluations, care plans, and progress notes, ensuring compliance with regulatory standards.

Your role will primarily involve working with a diverse population, including older adults, individuals recovering from surgeries, and patients managing chronic illnesses. Our focus on personalized care allows you to build meaningful relationships with clients and their families, ensuring they feel supported throughout their recovery journey. Your assessments and treatment plans will be integral to enhancing their mobility, independence, and overall well-being.

To be successful in this role, you will need the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A minimum of one year of professional experience as a physical therapist, preferably in an acute care or home care setting.
• A valid physical therapy license in good standing in New Jersey.
• Current CPR certification and a valid driver's license with automobile insurance.
• A strong commitment to clinical excellence, quality care, and integrity in practice.
• A proactive approach to patient care, including effective communication and problem-solving skills.

Although not mandatory, candidates with experience in home health physical therapy, knowledge of OASIS documentation, and familiarity with physical therapy devices will be at an advantage.
