Key Responsibilities
  • Direct physical therapy treatment to prevent disability, relieve pain, and improve functional capabilities in alignment with individualized plans of care.
  • Evaluate patients' functional levels through comprehensive assessments, including diagnostic tests on muscles, nerves, and joints.
  • Oversee the implementation of therapy plans by physical therapy assistants to ensure quality treatment delivery.
  • Make recommendations to optimize the home environment for patient rehabilitation and independence.
  • Administer therapeutic exercises and treatments, supporting patients through hands-on guidance and motivational strategies.
  • Participate actively in developing, reviewing, and updating client care plans, ensuring they reflect current goals and therapeutic needs.
  • Document all necessary clinical records, including evaluations, progress notes, and discharge summaries, adhering to relevant policies.

Qualifications
  • A Bachelorโ€™s Degree in Physical Therapy from an accredited program.
  • A minimum of one year of experience as a licensed physical therapist in either an acute care or home care setting.
  • Current active Physical Therapist licensure in good standing in the state of New Jersey.
  • CPR certification and a valid driverโ€™s license with automobile insurance.
