Key Responsibilities

In this role, you will be responsible for:

  • Documenting patient progress meticulously through clinical records, including evaluations, care plans, and discharge summaries, ensuring compliance with organizational and regulatory standards.
  • Participating in interdisciplinary team meetings to discuss patient cases, share insights, and collaboratively revise treatment plans based on observed results and patient feedback.
  • Providing education to patients and their families regarding physical therapy practices and the use of assistive devices, ensuring they feel informed and empowered throughout their rehabilitation journey.
  • Maintaining timely documentation and communication with physicians and case managers regarding any changes in patient conditions or responses to therapy.
