Job description

PHCA is a start-up and approved ODP Provider. PHCA provides supports to adults who has intellectual disabilities, specifically in the Family Services Spectrum

MUST BE AVAILABLE TO START IMMEDIATELY.

Must have collegiate credits AND a copy of resume of professional experience as a program specialist in residential or community settings - working with intellectual disability, developmental disability, autism, or serious mental illness. Must have experience with HCSIS and Therap.

Must possess extensive knowledge on Everyday Lives' Principles and PHCA's mission, values and vision!!!!

Must have excellent writing, analytical, presentation, clinical and interpersonal skills. Excellent clinical, communication and interpersonal skills, excellent writing skills, thorough knowledge of Intellectual Disabilities Field and Best Practices. Valid driver?s license and vehicle required.

Location: Virtual and In-Office

Schedule: Monday - Friday 9:00am 5:30 pm (must be available by phone, email or text during the weekends) - Salaried, hours may extended based on the discretion of management

Specific Duties:

  • Provide supports for on-boarding to include access to Therap or other softwarre  for new employees/contractors, reviewing progress notes, entering ISP information into Therap, mobile app
  • Scheduling
  • Post Requisitions / Job Postings
  • Maintain and enforce compliance with DPW/ODP regulations.
  • Training
  • Conduct Site Orientation of new staff/Contractors to assure that staff are knowledgeable about individuals' needs/services and ensure that they are aligned with the individuals' preferences and Everyday Lives Principles
  • Regularly observe and provide feedback/training to Direct Support Professionals, Program Specialists, and Supervisors to ensure that their interactions with each individual are consistent with the individual?s goals, uses techniques and approaches that positively supports individual?s ISP, as well as ODP?s Everyday Lives Philosophy
  • Appropriately schedule and ensure proper usage of agency/program vehicles
  • Participate in Human Resource related issues including disciplines, write ups, investigations, and coordination of staff drug testing
  • Exclusion lists
  • Complete all required ODP trainings on time
  • Inform PHCA's supervisory staff of all relevant issues consistent with PHCA and City/State Policies to assure the health and safety of those individuals and staff who we serve
  • Reconcile electronic time keeping exceptions daily, approve time sheets, work in conjunction with the scheduler to assure that all shifts are appropriately provided and the health and safety of those in serve are maintained.
  • Assure the completion and processing of Incident Reports occurs within PHCA established time lines, preferably same day EIM entries
  • Work closely with Program Supervisors to coordinate and provide quality supports
  • Assist in individuals? celebrations and client related issues, such as but not limited to Birthday/ retirement events, voter registration and funeral planning, if applicable
  • Ensure staff provides all clearances, completes all tests and training, complete 8850
  • Ensure Front desk has sign in sheet for interviews
  • Providing oversight to Front Desk
  • Maintaining ODP Record Documentation
  • Compiling daily notes into monthly notes, into quarterly notes
  • Therap actions such as assigning staff to admitted individulas
  • Sending  welcome packets 
  • Other duties Assigned

Job Type: Full-time

Salary: $15.00 to $19.00 /hour

Experience:

  • working with adults with intellectual disabilities: 1 year (Preferred)
  • training: 1 year (Preferred)
  • developing activity calendar: 1 year (Preferred)
