Job description

Weekend Registered Nurse (RN)

Facility: Senior Care Partners P.A.C.E.
Location: Battle Creek, MI 49017
Contract Length: 13 Weeks
Schedule: Saturday & Sunday Only
Shift: 7:00 AM โ€“ 11:00 PM
Pay Rate: $46 - $51/Hour

Primary Purpose

The Weekend RN is responsible for supporting the mission and core values of Senior Care Partners PACE by conducting participant assessments, coordinating care, providing nursing services in the field, and ensuring participant-centered care for the frail and elderly population.

Key Responsibilities

โ€ข Perform in-person participant assessments to determine the appropriate level of care.
โ€ข Respond promptly to calls from PCPs, triage staff, and support teams.
โ€ข Document all participant interactions and follow-up activities.
โ€ข Complete home visits and provide nursing care as ordered, including:

  • Wound Care
  • IV Hydration
  • Lab Draws
  • Nursing Assessments
  • Other Skilled Nursing Services
    โ€ข Educate participants, families, caregivers, and facility staff.
    โ€ข Coordinate care with providers, pharmacies, hospitals, home care teams, ALFs, SNFs, DME suppliers, and other partners.
    โ€ข Initiate emergency treatment when necessary.
    โ€ข Process and document physician orders and care plan updates.
    โ€ข Complete incident reports related to falls, medication errors, grievances, and quality measures.
    โ€ข Participate in staff meetings, quality improvement initiatives, and ongoing training.
Qualifications

โ€ข Associate Degree in Nursing required.
โ€ข Bachelor's Degree in Nursing preferred.
โ€ข RN Diploma or Degree from an accredited School of Nursing.
โ€ข Minimum 1 year of experience with frail, elderly, or long-term care populations.
โ€ข Home Care experience preferred.
โ€ข Wound Care experience preferred.

Required Certifications

โ€ข Current RN License
โ€ข CPR Certification (AHA) or ability to obtain upon hire

Additional Requirements

โ€ข Reliable transportation required.
โ€ข Valid Michigan Driver's License and current auto insurance.
โ€ข Must be at least 18 years old.
โ€ข Must be fully vaccinated (or willing to become fully vaccinated) for:
