We are looking for an energetic and self-motivated Registered Nurse to join our care team. You will be responsible for providing ongoing support to patients while they transition from the hospital and for educating patients on managing their chronic conditions. The ideal candidate will be able to work collaboratively with a team of doctors, advanced practice providers, pharmacists and dieticians to promote patient well-being.

Responsibilities

  • Collaborate with physicians and advanced practice providers to develop individualized care plans for patients
  • Care management encounters including hospital transition coordination, medication reconciliation and chronic condition management
  • Engage in quality-of-care programs, especially with high-risk patients
  • Assist patients with needed resources
