Job description

The schedule for this position is Monday-Friday 8:00am-5:00pm; available one weekend shift per quarter; one major or two minor holidays per year.
Hiring for Wake County- Cary/ Apex Area.

Transitions LifeCare seeks to hire full-time Community-based Hospice RN Case Manager who embraces our mission of caring for individuals and families with compassion, support, and expertise as they navigate the end of life or life-changing illness.

A Successful RN Case Manager has the following:

  • Caring and compassionate attitude with true concern for people
  • Excellent interpersonal skills: ability to connect with patients, families, and internal/external partners
  • Strong ability to problem-solve and communicate complex issues; ability to work collaboratively with all co-workers
  • Exhibit ability to make responsible choices and decisions that are in patients’ best interest

Responsibilities:

  • Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients
  • Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
  • Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
  • Maintain comfort, privacy, and dignity of our patients
  • Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance

Requirements:

  • Registered Nurse with current NC license; BSN preferred.
  • Must have reliable transportation to travel to enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
  • Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
