In collaboration with the medical and facility staff, coordinates multidisciplinary care for the Stroke and Sepsis patients. Works in tandem with the Medical Directors of the Stroke and Sepsis program. Roles and responsibilities include but are not limited to the following:
  • Assists in maintaining Stroke certification and accreditation.
  • Reviews and revision of administrative and patient care policies, procedures, and guidelines related to the care of the Stroke or Sepsis patient.
  • Attends interdisciplinary rounds to facilitate the delivery of evidence-based care and best practices.
  • Facilitates collaborative interdepartmental practices from patient admissions through discharge and community re-entry.
  • Uses critical thinking skills, leadership, to anticipate, plan, direct and evaluate the care of stroke and sepsis patients provided by a team of physicians, registered professional nurses and ancillary personnel.
  • Collaborate with other health team members to provide coordinated, safe, and compassionate care for stroke and sepsis patients.
  • Co-chairs Stroke and Sepsis Committees, assist with analysis and process improvement efforts regarding care of the inpatient stroke victim.
  • Ensures that all regulatory requirements are met to achieve and sustain stroke certification and Sepsis measurements which includes data abstraction.
  • Subject-matter expert on stroke and sepsis clinical practice guidelines providing education to staff and physicians on an ongoing basis regarding stroke and sepsis care protocols and assure competencies.
  • Focuses on integrity of medical record documentation through appropriate interactions with physicians, nursing staff, and other patient caregivers.
  • Serves as the liaison between the hospital and the community in providing education related to the stroke and sepsis program including stroke prevention, signs and symptoms, risk factors, and research.

Minimum Qualifications: Bachelor of Science in Nursing, masterโ€™s degree (preferred). Minimum of three (3) years of relevant experience in acute care setting overseeing and/or performing data abstraction, analysis, and process improvement. Basic Life Support - BLS/BCLS (American Red Cross or American Heart Association) -

Course/Certification (required) Advanced Life Support - ALS/ACLS - Course/Certification. Certification in emergency nursing preferred.

Current experience in caring for neurology patients, Current National Institute of Health Stroke Scale (NIHSS) Certification and 8 hours of stroke education annually required.

Knowledge, Skills amp; Abilities: Knowledge of clinical documentation systems/EHR. Experience in a quality improvement department setting preferred. Detail-oriented with strong organizational and analytic problem-solving skills, with exceptional verbal and written communication skills. Competent in the use of Microsoft Office Word and PowerPoint, and Excel. Knowledge of, or experience in, the delivery of patient care needed to perform the duties of the position. Experience interacting with regulatory surveyors preferred. Maintains knowledge of measure specifications and data definitions and facilitates process improvements to meet requirements.
