Job description

Works under the supervision of the Manager/Director of Home Care. Assesses, plans, implements, evaluates, and supervises individual patient care to patients in their place of residence, according to policies and procedures. Accepts the responsibility for the direction of co-workers in the implementation of care plans. Coordinates all home care services provided by the home health agency.
Education, Work Experience and Licensure:
  • Graduate of an approved Nursing Education Program.
  • Holds a current nursing license in good standing for Ohio and West Virginia. Note: WV license required for all full time and part time nurses working out of the Pomeroy and Gallipolis areas. WV license may also be required for the per diem RN positions, depending on which area the per diem RN is hired for, and if the per diem nurse will be taking call. For per diem nurses hired with the understanding that the position requires on call coverage, dual licensure is required for the Pomeroy and Gallipolis areas
  • Minimum of one year nursing experience required.
  • Six months to one year home health experience desired.
  • Previous experience preferred.
  • Valid driver's license, proof of auto liability insurance and clean driving record. A clean driving record includes, but is not limited to the following: no OVI, no reckless operation, and no failure of physical control on driving record. If such violations should occur after employment, employee may not continue working in the Home Care department. Citations will be reviewed on a case by case basis.
  • No criminal record as validated by criminal background check, including fingerprinting.
