Job description

We're seeking a Registered Nurse to support our dedicated Home Health team in an admissions and float capacity.
Shift: Per Diem
Our service area covers King County. In this rewarding position, the physical therapist works collaboratively with other Home Health team members to admit patients to service and develop a comprehensive plan of care that meets the needs of patients and their families. Care settings include seeing patients in their private residences, adult family homes, and assisted and independent living facilities.
Some responsibilities include:
* Comprehensive patient assessment
* Developing the care plan in collaboration with the patient and their family
* Admission tasks to include: education, coordinating resources, communicating with physicians and other team members, pain and symptom management and skilled physical therapy interventions.
* Expand and develop OASIS knowledge/expertise.
* Home Health experience Strongly Preferred
* Active WA RN license
* OASIS experience
* 1 year experience working in Admissions
* Valid driver's license, auto insurance and dependable vehicle
* Computer skills for charting and documentation
