Job description

Position Summaryย 

The Per Diem Registered Nurse (RN) at NOHN supports the Nursing Services Department in a variety of areasย in an ambulatory care setting. The per diem nurse may provideย services includingย clinical triage, direct patient care, medication management, transitional care management, vaccinations,ย orย care coordination. The RN works closely with providers, patients, pharmacies, and community agencies to support high-quality patient care, improve outcomes, and ensure continuity of care. This role may also provide direct patient visitsย for specialized servicesย including wound care and Office-Based Addiction Treatment (OBAT) support, based on clinic needs and competency.ย ย 

This position offers a flexible opportunity for an RN to support patient care across triage, care coordination, medication management, transitional care, and direct clinical services in a collaborative outpatient setting.ย 

Essential Duties and Responsibilitiesย 

Clinical Triageย 

  • Perform telephone and in-person patient triage using established protocols and clinical judgment.ย 

  • Monitor EMR messages and respond to urgent patient needs.ย 

  • Collaborate with providers and care teams to ensureย timelyย patient evaluation and treatment.ย 

  • Coordinateย appropriate schedulingย and follow-up based on patient needs.ย 

  • Serve as a clinical resource for staff and escalate urgent concerns to providers.ย 

Medication Managementย 

  • Process andย monitorย medication refill requests.ย 

  • Verify patient information, medication history, and refill eligibility.ย 

  • Communicate with pharmacies and providers to resolve medication-related issues.ย 

  • Maintainย accurateย medication records in the EMR.ย 

  • Educate patients on medication use, adherence, and follow-up care.ย 

Transitional Care Management (TCM)ย 

  • Conduct post-discharge outreach and assessments following hospitalization or emergency department visits.ย 

  • Review discharge instructions, medications, and care plans with patients.ย 

  • Coordinate follow-up appointments, referrals, and community resources.ย 

  • Monitor patient progress and communicate concerns to providers.ย 

  • Documentย transitional care activitiesย in accordance withย regulatory requirements.ย 

Direct Patient Careย 

  • Provide nursing visits and clinical services withinย scopeย of practice.ย 

  • Perform wound assessments, wound care treatments, and patient education asย appropriate.ย 

  • Support Office-Based Addiction Treatment (OBAT) programs, including patient education, monitoring, care coordination, and collaboration with prescribing providers.ย 

  • Administerย medicationsย and complete required documentation.ย 

  • Assistย with other clinical procedures and patient care activities as assigned.ย 

Documentation and Care Coordinationย 

  • Maintainย accurate,ย timely, and complete documentation in the electronic medical record (EMR).ย 

  • Coordinate care with providers, specialists, pharmacies, hospitals, and community agencies.ย 

  • Ensure compliance with HIPAA, nursing standards, organizational policies, and regulatory requirements.ย 

  • Participate in team meetings, quality improvement activities, and other duties as assigned.ย 

Qualifications:ย 

  • Education:ย Graduate of an accredited school of nursingย required. BSN preferred.ย ย 

  • Experience:ย Current, active WA State Registered Nurse licenseย required. Certified Basic Life Supportย required.ย ย 

  • Skills:ย Proficiencyย in Electronic Health Record systems, Ability toย utilizeย computers for data entry and information retrieval, excellent verbal and written communication skills,ย Demonstrateย a high levelย of skill at building relationships and customer service. Demonstrate influential skills in managing difficult clients and patients. Demonstrate a high degree of knowledge and competency in the practice of medicine and associate charting requirements. Requisite skills and ability to perform certain medical tasks as assigned. Demonstrateย a high levelย of problem-solving skills to better serve patients and staff. Strong organizational skills, ability to workย independentlyย and as part of a team, cultural sensitivity. Ability to implement and evaluate operational and administrative processes.
