LPN Per Diem

We are seeking a dedicated and compassionate Per Diem Licensed Practical Nurse (LPN) to join our team at Chestnut Park Rehabilitation & Nursing Center in Oneonta, New York. Our facility is a 80-bed sub-acute rehabilitation and nursing center nestled in the beautiful Catskill Mountains, offering a relaxed and peaceful environment to our residents.

Our Per Diem LPN will provide nursing coverage as needed across all shifts, including day, evening, and night shifts. This role is ideal for nurses seeking maximum flexibility, supplemental income, or limited commitment. As a valued member of our interdisciplinary team, you will work closely with our staff to deliver high-quality care to our residents.

Responsibilities:

  • Administer medications and treatments as prescribed by physicians

  • Conduct resident assessments and monitor their conditions

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Accurately document resident care and clinical information

Requirements:

  • Active NYS LPN license

  • CPR certification

  • Prior long-term care experience preferred
