Per Diem Field RN Home Health Visits for Medicare

Are you an experienced RN with a passion for providing exceptional care? AccordCare Home Health is seeking skilled Per Diem Field RNs to support our Medicare Home Health Care patients. If you have current OASIS experience and a commitment to quality care, we'd love to have you on our winning team!

Deliver safe, high-quality care based on individualized Plans of Care

Communicate promptly with the team regarding patient status and changes

Provide skilled nursing (e.g., wound care, infusion, catheter care, medication administration)

Partner with Nurse Supervisor/Manager of Clinical Practice to ensure optimal patient outcomes

Show empathy, professionalism, and excellent follow-through with patients and families

Current RN license in Florida

Home Health experience, or willingness to learn

OASIS experience, or willingness to learn

Strong clinical skills

Excellent communication, collaboration, and documentation skills

Familiarity with WellSky/Kinnser/AlayaCare is a plus
