Our organization provides services designed to help members of our community achieve their individual goals. MPNN BH has over 60 distinct programs to aid and support all ages and many situations.
Job Summary
The Jail Diversion Case Manager will provide specialized services to adults involved with the criminal justice system to reduce unnecessary incarceration and improve access to community-based mental health care. This role operates across MPNN BH locations, local correctional facilities, and community settings. The Case Manager will collaborate with correction facility staff, law enforcement, court personnel, treatment providers, and other community resources to implement diversion strategies, support individuals in connecting with mental health and support services, and monitor outcomes to ensure program goals are met.
Job Responsibilities
  • Provide direct case management and support to a target population of at least 28 adults during the fiscal year, as outlined in program performance goals.
  • Conduct assessments and develop individualized service plans for clients involved in or at risk of involvement with the criminal justice system.
  • Coordinate with local correctional facilities, courts, law enforcement, and community agencies to facilitate diversion from incarceration.
  • Assist clients in accessing mental health treatment, social services, housing, and other community resources.
  • Monitor client progress and provide ongoing support to ensure engagement in services and reduction of recidivism.
  • Maintain accurate and timely documentation of client interactions, assessments, and services provided.
  • Prepare quarterly and annual reports in accordance with grant reporting requirements, including narrative and financial data submission deadlines (Q1: Nov 15, Q2: Feb 15, Q3: May 15, Q4: Aug 15).
  • Participate in team meetings, trainings, and other professional development opportunities.
  • Ensure compliance with all applicable policies, procedures, and confidentiality requirements.
  • Perform other related duties as required.

Requirements
Job Requirements
Essential
• Must pass all required Background Checks.
• Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
• Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority.
  • Knowledge of community-based mental health resources and interventions.
  • Strong case management, assessment, and documentation skills.
  • Ability to work independently in multiple settings, including correctional facilities and community environments.
  • Excellent interpersonal, communication, and problem-solving skills.

Education and Experience
  • Bachelor's degree in Social Work, Psychology, Counseling, or related field required; Master's preferred.
  • Experience in criminal justice, mental health, or social services preferred.
  • QMHP-T eligibility or QMHP certification preferred.
