Position Summary

The Intake Coordinator is responsible for providing administrative and clerical support to the Intake Department. This role assists with coordinating intake activities, maintaining records, supporting scheduling functions, and ensuring the efficient flow of information between departments.

In addition to these responsibilities, the Intake Coordinator plays an active role in identifying, nurturing, and converting qualified referral opportunities into admissions. This position offers a performance-based commission opportunity for successfully converted leads, providing additional earning potential while supporting agency growth.

Primary Responsibilities

  • Provide administrative and clerical support to the Intake Department.

  • Assist with coordinating daily intake and scheduling activities.

  • Maintain accurate records, logs, and departmental documentation.

  • Support the collection, organization, and distribution of information related to patient admissions and services.

  • Assist with monitoring schedules and communicating updates as needed.

  • Handle incoming telephone calls and route inquiries appropriately.

  • Communicate with internal departments and external contacts to obtain or relay information.

  • Assist with data entry and maintaining information within agency systems.

  • Support departmental projects and process improvement initiatives as assigned.

  • Ensure confidentiality and accuracy of patient and agency information.

  • Perform general office duties, including filing, scanning, copying, and document management.

  • Provide additional administrative support and perform other duties as assigned by management.

  • Identify, qualify, and follow up on prospective patient referrals to facilitate admissions.

  • Track lead activity, follow-up efforts, and conversion outcomes within agency systems.

  • Earn performance-based commission incentives for qualified leads that successfully convert to admissions, in accordance with the company's incentive plan.
