POSITION SUMMARY

The Care Partner provides direct resident care, completes assignments as a universal worker, and assists in maintaining a positive physical and psychosocial environment for residents. The Care Partner assists with delegated tasks and medication management, as well as other tasks needed for resident well-being and building support.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  1. Provides direct resident care while promoting resident dignity, independence, and personal choice. Regularly observes residents' physical and mental conditions and reports changes to health services leadership or Executive Director.
  2. Provides care and services as directed by the resident's cooperative service plan.
  3. Coordinates personal care services with co-workers assisting with lifting and responds to assistance pages promptly.
  4. Reports and documents all changes of condition, incidents, and accidents per policy. Completes are required notifications and incident report forms.
  5. Performs First Aid and initiates emergency response procedures as required.
  6. Greets residents, visitors, and staff courteously and answers phones when assigned.
  7. Follows and supports community fire safety and infection control practices and promptly reports any unsafe conditions or equipment to immediate supervisor or Executive Director.
  8. Serves meals in the dining room, including meal set up, service, assisting residents at mealtime, provides resident tray delivery, and assists with dining room and kitchen clean up and maintenance.
  9. Assists residents with keeping their apartments clean. Communicates observed problems appropriately.
  10. Partners with community team to clean all common areas of the building.
  11. Assists with laundry service for residents, as well as building laundry.
  12. Follows the daily schedule and the planned activity program schedule. Supports and encourages residents to participate as is appropriate for their abilities. Assists residents to and from activities.
  13. Administers resident medications and treatments according to scheduled times; following the medication administration policies, as assigned and delegated.
  14. Assists with tours, move-ins, move-outs, and transfers.
  15. Assists with apartment housekeeping services and resident's laundry.
  16. Attends and participates in training, in-services, and staff meetings. Completed all required training.
  17. Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE
  • Must hold state required certification, complete state required delegation and/or training requirements and successfully complete the ADL competency demonstration. Must have word processing skills. Current First Aid and CPR certification. Food Handlers permit if required by State.
EDUCATION
  • High school diploma or GED with one or more years of personal care experience with older adults preferred or equivalent knowledge and skills obtained through a combination of education, training, and experience.
