Opportunity:

RN - Home Health Nurse

Job Details:

• Work 8-hour day shifts from 8:00 AM to 4:30 PM, Monday through Friday
• Rotating weekend and holiday schedule required
• One weekend on-call per month and one evening on-call per week
• Expected to work 5 days per week unless scheduled for a rotating weekend, in which case a day off before and after will be provided
• Typical day involves 6 points of care, including an admission plus 3-4 patients
• Required to perform Start of Care (SOC), Re-certifications, and Resumptions
• Care for Home Health and Hospice patients with diagnoses including CHF, Diabetes, Renal Disease, Wound Care, IV therapy, and management of drains and other devices
• Recent Home Health experience within 2 years required with proficiency in OASIS documentation
• Wound care experience and excellent infection control practices in the home environment required
• All nurses will work both Home Health and Hospice; primary hospice patients typically not assigned to travel nurses but may be required to see hospice patients while on call
• CNA support available on site
• Manager, RN coordinator, and office support supervisor available after hours as needed
• Dress code: Any solid color, clean and professional scrubs
• Required certification: BLS (AHA)
