Job description

Luther Park Community is looking for a part-time social worker to assist the Director of Social Services at the Trinity Center, a 120-bed skilled nursing facility which specializes in Alzheimer's care.
JobSummary:
The part-time social worker isresponsible for assisting in meeting and assisting residents in attaining ormaintaining their highest practicable well-being. This includes identifying the need for medically-related social services and ensuring that these servicesare provided in accordance with State and Federal regulations.
EssentialFunctions:
Complete or delegate the completionof the social services component of the comprehensive assessment and contributeto and/or direct/delegate contribution of social services goals and approachesto the comprehensive care plan. These goals and interventions will beindividualized to match the skills, abilities, and interests/preferences of each residentin compliance with Federal and Stateregulations, to include identifying and promoting individualized,non-pharmacological approaches to care that meet the mental and psychosocialneeds of each resident.
Facilitate residents' safe transitionback into the community through interdisciplinary dischargeplanning and arrangement of community based services and follow-up care. Assist residents and their representatives in locating and accessing financial, legal, and othercommunity resources.
Coordinate implementation andoversight of procedures to ensure social servicesactions and interactions are adequately documented in each resident's medical record, and that legal,ethical, and professional standards of social work practice are being upheld inwritten recordings.
Assist the Social Services Directorin overseeing the establishment of departmentalQA procedures and modification of those procedures where appropriate.
Assist in the process of Advance CarePlanning for each resident upon admission and make sure any Advance Directivesare reviewed with the resident/resident representative on a regularbasis. Ensure staff membersare made aware of the resident's code status and end-of-life wishes and assistwith informing and educating residents and their representatives about healthcare options and ramifications.
Participate in facilitypolicy development in order to positivelyimpact the quality of care delivered to residents.
Ensures that staff membersare knowledgeable about Resident'sRights and encourages staff to maintain and enhance each resident's dignity inrecognition of each resident's individuality. Advocate for residents and assistthem in assertion of their rights within the facility. Report any allegationsor suspected abuse, neglect or exploitation to the Director/Administrator andappropriate State agency and assist in a thorough investigation into theallegation.
Ensure residents who display mentalillness or psychosocial difficulties such as copingwith grief and loss, will have accessto appropriate treatment andresources.
Work alongside other departments toensure the facility is in compliance with TraumaInformed Care. Ensureall residents are assessedfor trauma upon admission and on a regular basis and that trauma andcorresponding interventions are reflected on the care plan.
Assist with the grievance process and educate staff, as well as residents andfamilies of the grievance process.
AdditionalAssigned Tasks
• Make arrangements for residents to obtain items such as clothing and personal care supplies.
• Act as a resource for staff participating inbehavioral interventions with residents.
• Participate in Resident and/or Family Council asneeded or requested.
• Assist with or arrange for, a resident's communication of needs through the resident'sprimary method of communication or in a language that the resident understands.
• Treat all residents with dignity and respect.Promote and protect all residents' rights.
• Establish a culture of compliance by adhering toall facility policies and procedures.
• Comply with standards of business conduct andstate/federal regulations and guidelines.
• Maintain confidentiality of protected healthinformation, including verbal,written, and electroniccommunications.
• Report noncompliance with policies, procedures, regulations, or breachesin confidentiality to appropriate personnel.
• Report any retaliation or discrimination to HRor the compliance officer.
• Report any occupational exposures to blood, body fluids,infectious materials, and/or hazardous chemicals in accordancewith facility policy.
• Report work-related injuries and illnessesimmediately to supervisor.
• Contribute to the facility effortsto maintain and/or improve qualityof care through participation in the following:
Attend and facilitate Care Plan meetings when requested.
Attend mandatory in-services.
Successfully completethe facility requiredtraining, and any Social Work continuing education and licensingrequirements.
Experience/EducationalRequirements:
A minimumof a bachelor's degree in social work or anotherhuman services fieldto include, but not limitedto, sociology, gerontology, special education, rehabilitation counseling, or psychology.
One year of supervised social work experience in a healthcare setting workingdirectly with individuals.
