Job description

Job Title: Licensed Practical Nurse- Home Care Team Case Assistant (Part Time)
Location: Rochester Regional Health Home Care - Monroe Ave, 330 Monroe Avenue, Rochester, NY 14607

Hours Per Week: 20

Schedule:Day/Evening shifts: Friday 12pm-5pm, Saturday 8am-5pm, Sunday 8am-4pm

SUMMARY:

Utilizing clinical experience, coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.

RESPONSIBILITIES:

  • Validates supplemental orders and processes per policy.
  • Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN.
  • Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers' needs are met.
  • Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee.
  • Checks email in a timely manner providing follow up to assure sender's needs are attended appropriately.
  • Collects, organizes and reviews various programs for information and reporting and initiates follow up.
  • Updates the EMR per agency standards.
  • Performs functions related to transfers or discharges as required by regulation.
  • Builds and maintains strong, communicative relationships with the team.
  • Provides exemplary customer service.
  • Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies.

REQUIRED QUALIFICATIONS:

  • Graduate of an accredited school of licensed practical nursing.
  • A valid NYS license as a Licensed Practical Nurse.

PREFERRED QUALIFICATIONS:

  • One year of experience in home care setting or medical office preferred.
  • Excellent interpersonal skills and customer service skills.
  • Ability to multi-task in a fast paced environment.
  • Proficient use of Microsoft office preferred.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
