CHN is seeking a Full or Part Time RN case manager for Wayne County.

Case Managers are responsible for completing OASIS assessments (Start of Care, Recertification, Discharge, Resumption of Care) as well as Skilled Visits, Med Passes, and Supervisory Visits. On-Call rotation required.
