Locations: Hamilton County: Cincinnati, Norwood, Blue Ash, Forest Park, Springdale and all other locations!

What You Will Do:
  • Conductย Start of Care (SOC)ย visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
  • Performย skilled nursing visits and ongoing reassessmentsย to monitor patient condition and adjust care plans as needed
  • Provideย hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
  • Deliverย patient and caregiver educationย on diagnoses, medications, and preventive care to reduce hospitalizations
  • Collaborate withย physicians and interdisciplinary team membersย to ensure coordinated, high-quality care
  • Maintainย accurate and timely clinical documentationย in compliance with state regulations and agency standards
  • Supervise and delegate careย to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
What You Will Have:
  • Active Registered Nurse (RN) license in the state of practice
  • Experience in home health, skilled nursing, or a related healthcare setting preferred
  • Strong clinical assessment and critical thinking skills
  • Knowledge of home health regulations and documentation standards
