Job description

Summary/Objective

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our General Residential Operations Program in Texas. The RN will be responsible for providing high-quality healthcare to residents, monitoring their health status, and ensuring a safe and supportive environment. Directs and oversees all resident health/medication related services in conjunction with the client’s medical consenter and as indicated by the resident’s service plan. The ideal candidate will have strong clinical skills, a patient-centered approach, and the ability to collaborate with a team of healthcare professionals to deliver comprehensive care.

Required Education and Experience

·       Current, unrestricted Registered Nurse (RN) license in the state of Texas.

·       Experience with providing and coordinating services to care for and support clients with an emotional, conduct, and/or behavioral disorder

·       Ability to navigate and coordinate healthcare services (when needed)

  • Available both onsite and via phone/video conference

·       Strong knowledge of healthcare practices, including medication management, infection control, and wound care.

·       Ability to assess and respond to changes in residents’ conditions promptly and effectively.

·       Excellent communication skills, both verbal and written.

·       Compassionate, patient, and detail-oriented with a strong commitment to resident care.

·       Ability to work independently and as part of an interdisciplinary team.

·       Basic Life Support (BLS) certification required

·       Knowledge of relevant Texas state regulations and healthcare standards.

Preferred Education and Experience

Essential Job Functions

General

·       Provide direct nursing care to residents, including administering medications, treatments, and performing medical procedures as ordered by physicians.

·       Monitor residents’ physical and emotional well-being, assessing for changes in condition and responding to health concerns.

·       Develop and implement individualized care plans for each resident, in collaboration with physicians, caregivers, and other healthcare professionals.

·       Conduct regular health assessments, including vital signs, wound care, and other medical evaluations.

·       Administer and document medications, treatments, and therapies in compliance with physician orders and program policies.

·       Provide health education to residents and the staff, ensuring they understand care instructions, treatments, and health conditions.

·       Coordinate with other team members to ensure residents receive comprehensive care, including physical, emotional, and psychological support.

·       Ensure compliance with all regulatory standards and maintain up-to-date medical records in accordance with state and federal guidelines.

·       Respond to medical emergencies as needed and provide immediate care, including first aid, CPR, and emergency medical services until further assistance arrives.

·       Advocate for residents’ health and safety, ensuring they are treated with dignity and respect at all times.

·       Participate in regular team meetings, providing input and suggestions to enhance the quality of care.

Treatment Model & Training

  • Trains and educates staff, the client’s family and/or other supportive persons, and DFPS/SSCC staff (as needed) on client specific medical needs (including health needs, medication administration, psychotropic medications, etc.)

Nursing Services

  • Participates in STAR Health Service Coordination (when needed)
  • Provides ongoing consultation to staff, the client’s family and/or other supportive persons, and DFPS/SSCC staff (as needed) on client health/medication needs

·       Reviews client records including health related documentation and Medication Logs

·       Seeks out community resources to obtain needed services that are not covered through STAR Health (when needed)

·       Participates in Continued Stay Evaluations (when necessary)

·       Documents all health services requested, denied, and/or provided

Continuous Quality Improvement

·       Continuously review and update individualized care plans by collaborating with healthcare professionals

·       Regularly audit medical records, medication logs, and health assessments to ensure compliance with regulatory standards

Work Schedule

The facility is a 24-hour facility. Positions may require work on holidays, weekends, or both depending on when days fall.

Work Environment

The RN position is exposed to youth with complex histories of trauma, abuse, and neglect, as such there will be situations where residents may become emotional or hostile. This role uses standard office equipment and requires the use of computers, phones, copiers, filing cabinets, emails, medication, and fax machines.

Physical Demands

The employee is occasionally required to sit; climb or balance; and stoop, kneel, crouch or crawl. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. As a last resort, the RN may be required to perform physical restraints utilizing provided training techniques. 

Travel

Occasional travel is expected for this position, but is generally local.

Work Authorization/Security Clearance

●      Driver’s License. Drivers must have a valid Texas Driver's License with a driving record that would not prevent them from being insured by the company vehicle insurance carrier. Drivers must also maintain this status throughout their employment in this position as the position requires driving youth as a critical function.

●      Authorized to Work in the United States. Azleway requires all employees to lawfully reside in the United States of America and be authorized to work.

●      Must be able to pass a background check, drug screen, TB test, and driving record check.
