Job Description
White Oaks Rehabilitation & Nursing Center is seeking an experienced Nurse Risk Manager for direct hire. The Risk Manager is responsible to mitigate patient health risks and to develop and implement policies and processes to ensure the facility’s safety and accountability. The Risk Manager is obligated to ensure that all occurrences/incidents within the SNF are thoroughly investigated and reported according to DOH and policies and guidelines. Additionally, the Risk Manager will work with interdisciplinary teams to assure the effectiveness of the program, and educate and counsel staff. The Risk Manager is an important member of the leadership team. A competitive salary, excellent benefits, 401K, and other perks are offered. Responsibilities: • Investigate, Determine, and Report (using Occurrence Logs) all Incidents according to the NYS DOH Incident Reporting manual.
• Assess liability and financial risks (falls, medication errors, abuse and neglect, hazardous conditions, etc.)
• Report on all ongoing investigations to Administrator, DNS, and IDT
• Implement mitigation strategies, assess their effectiveness, and make revisions accordingly.
• Educate and counsel staff. Run Safety Meetings.
• Initiate and direct all investigations and notifications to authorities.
• Issue timely reports to regulatory agencies and authorities.
• Represent the facility at all audits, inspections, and investigations by governing authorities. Requirements: • At least 5 years of RN nursing experience. Valid NY RN license
• At least 2 years of experience in long-term care or Skilled Nursing facilities - a must. Solid understanding of DOH and State regulatory compliance
• At least 2 years of Supervisory experience.
• Experience with assessing risks and financial liability
• Experience with writing and implementing policies and procedures • Ability to work with interdisciplinary teams, counseling and educating on risk mitigation
• Ability to conduct investigations and write and present reports.
Compensation commensurate with Nursing License and Experience
