Job description

CLINICAL QUALITY MANAGER
Job Description:
The Clinical Quality Manager (CQM) is a registered nurse who works full time and has the authority and responsibility to take reasonable steps to identify and implement strategies to foster the control of conditions and behaviors, which may expose the organization, facility, and staff to the possibility of risk or financial loss. In conjunction with the Administrator and Director of Nursing, the CQM will work to ensure that resident care objectives are met, consistent with the standards, policies, and procedures of the facility, and current standards of care and practice. The CQM is to identify potential/actual risk, which could cause or has caused harm to residents, staff and visitors. The CQM will monitor closely certain areas of resident care, establish training, and implement programs to decrease the possibility of risk. The CQM reports directly to the Quality Management Coordinator.
Essential Job Duties:
  • Provides development guidance and assists in the implementation, and monitoring of resident care objectives, clinical standards, policies, and procedures of Department of Nursing Services consistent with the standards, policies, and procedures of Grace Living Centers, the Principals of Respect and Statement of Commitment, and current standards of care and practice.
  • Embraces Grace Living Centers' philosophy to render care in a manner and in an environment that promotes maintenance or enhancement of each resident's clinical status and quality of life.
  • Establishes with the facility Administrator and Director of Nurses (Administrative Team) a mechanism to monitor the delivery of care, to facility residents consistent with state, federal and facility standards of practice.
  • Monitors systems of care, objectives, policies, and procedures, staffing patterns and staff orientation based on the needs of the resident. Identifies potential and actual risks of the facility's residents, staff, and visitors.
  • Monitors the delivery of care, documentation of care provided, and at times may deliver care consistent with the most current standards of nursing care and nursing practice. Communicates findings with the Quality Management Coordinator and the Administrative Team of the facility.
  • Encourages, teaches and motivates staff to perform resident care functions in ways designed to promote and increase the resident's overall quality of life while reducing the possibility of facility loss due to litigation.
  • Conducts daily rounds to identify potential or actual risks, deficient nursing practices, and documentation. Designs and implements training programs to reduce identified risks.
  • Meets regularly with the Administrator, Director of Nursing, and Quality Management Coordinator to address potential or actual risks, deficient nursing practices, and documentation. Offers recommendations to reduce or eliminate the potential or actual risk.
  • Works cooperatively with Director of Nursing and Administrator in the implementation of practices to reduce or eliminate potential or actual risk.
  • Establishes and administers a process for receiving, documenting, tracking, investigating and taking action on all complaints or reports of potential liability risks.
  • Investigates areas of identified risk and recommends corrective actions to reduce the possibility of financial loss.
  • Identifies educational opportunities for staff members. Develops and delivers in-service education according to identified needs.
  • Serves as a member of, or liaison to, the Corporate Compliance Committee, should one exist. Also serves as the information privacy liaison for users of clinical and administrative systems at the facility under the direction of the Privacy Officer.
  • Maintains current knowledge of applicable federal and state laws and Medicare certification standards, and monitors compliance.
  • Serves as clinical consultant to the facility for all clinical staff.
  • Works with Quality Management Coordinator, the Administrative Team, legal counsel, and other related parties to represent the facility's interests with external parties (state or local government bodies) who undertake audit, survey or otherwise monitor the facility's compliance to applicable law, regulation, or standard.
  • Performs other duties as assigned by the Director of Nurses, Quality Management Coordinator or the Administrator.
Job Knowledge & Skill:
  • Must possess comprehensive knowledge of good nursing practice and its application to the aged, chronically ill, and convalescent resident.
  • Must have a general knowledge of the Medicare/Medicaid program and must have knowledge of current federal and state rules and regulations for skilled (SNF) and nursing (NF) facilities, including but not limited to the Nursing Home Care Act, Oklahoma Nurse Practice Act, and OSHA's Bloodborne Pathogen Standards.
  • Must be familiar with facility policies and procedures and work to ensure that these policies and procedures are carried out in all aspects of resident care.
  • The CQM must possess knowledge of the legal process and have the ability to identify risk of liability.
  • Must possess skill in interpersonal relationships and leadership ability.
Qualifications & Education:
  • Must be licensed as a Registered Nurse in the State of Oklahoma and must remain in good standing with the Oklahoma Board of Nursing.
  • Must maintain his/her nursing license in current status and provide evidence of renewal as required by policies and procedures.
  • Two years prior experience in long-term care, knowledge of risk management and legal issues, and direct resident care is preferred.
  • Must demonstrate a moderate level of computer literacy in order to use and access our systems and tools within our facilities to complete online tasks and documentation.
  • Must be able to effectively communicate and interact professionally with staff and residents.

Physical Requirements
  • May be required to lift, carry, turn, or assist high-risk residents.
  • Must be able to sit, stand, bend, and move intermittently during work hours.
  • Must be able to lift, push and carry 40 pounds with 2 hands. Must be able to lift, push and carry 20 pounds with 1 hand.
  • Must be willing and able to work flexible work schedule.
