The Registered Nurse (RN) Case Manager will be responsible for providing comprehensive case management services to our clients, ensuring they receive the highest level of care and support. Responsibilities of a Registered Nurse (RN) Case Manager:

  • Complete new client admissions, including conducting initial assessments, obtaining medical histories, and developing individualized care plans.

  • Meet with families and clients to discuss the benefits of home care and address any questions or concerns they may have.

  • Develop the initial care plan based on client needs and preferences
