Your Role as a RN:
We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as
part-time, with flexible scheduling. Care locations would be in Davis & Weber Counties.
You'll work closely with an interdisciplinary team to:
  • Conduct admission and OASIS assessments
  • Perform head-to-toe and wound evaluations
  • Create and implement individualized plans of care
  • Provide follow-up skilled nursing visits
  • Educate patients and caregivers
  • Coordinate care with MDs, CNAs, PT/OT, and family members
  • Attend bi-weekly team meetings and contribute to care planning
