Responsibilities:
In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex
physical examination and psychosocial needs, self - management education & tools to better manage patient's multiple
conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary
care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure
development of care planning and access to preventive care services in accordance with member and Plan health outcome
goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.
Qualifications:
  • Master's Degree in Nursing; graduate of accredited Nurse Practitioner program
    required.
  • One year of clinical experience in role of nurse practitioner required; Minimum
    one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute and/or long term care setting
    preferred
  • Prior experience in managed care a plus
  • Ability to assess, diagnose, treat and follow-up
  • Ability to work independently and as part of interdisciplinary teams
  • Ability to develop relationships and communicate to members/family
  • Establish priorities and maintain schedules
  • NYS RN/NP License, Registration, DEA certificates
