Job description

Adult Foster Care (AFC) Program Job Openings

Program: MassHealth-Certified Adult Foster Care and Group Adult Foster Care Program
Schedule: Full-Time or Part-Time (Monday – Friday, flexible hours)
Travel: Required within local service area

Open Positions

Registered Nurse (RN) – Adult Foster Care and Group Adult Foster Care

Responsibilities:

  • Conduct initial and ongoing nursing assessments to determine eligibility and level of care.

  • Develop and update Individualized Service Plans (ISPs) and nursing care plans in collaboration with the Case Manager and participant’s primary care provider.

  • Perform home visits every 30 days to monitor health, safety, and caregiver performance.

  • Provide caregiver training and support on medication management, infection control, and emergency procedures.

  • Document all visits and participant updates in accordance with MassHealth AFC/GAFC regulations (130 CMR 408.000).

  • Work collaboratively with physicians, caregivers, and family members to ensure participant well-being.

Requirements:

  • Current Massachusetts RN license (in good standing).

  • Minimum 2 years of clinical nursing experience (home care, community health, or long-term care preferred).

  • Knowledge of MassHealth AFC Level I & II regulations and GAFC regulations.

  • Valid driver’s license, reliable transportation, and current CPR certification.

Case Manager – Adult Foster Care and Group Adult Foster Care

Responsibilities:

  • Conduct initial psychosocial assessments and assist in determining program eligibility.

  • Collaborate with the RN to create and maintain Individualized Service Plans (ISPs).

  • Conduct monthly home visits (every 30 days) to monitor participant adjustment and caregiver performance.

  • Provide emotional support, coordination of services, and advocacy for participants.

  • Document progress notes and updates per EOHHS and MassHealth AFC/GAFC standards.

  • Assist participants in accessing community resources and resolving social or environmental challenges.

Requirements:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field.

  • At least 2 years of experience in case management, home care, or human services.

  • Knowledge of MassHealth AFC/GAFC program regulations.

  • Excellent communication and organizational skills.

  • Valid Massachusetts driver’s license and reliable transportation.
