What you'll do
The Licensed Practical Nurse (LPN) Ambulatory Care and LPN, Advanced, provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician's Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN, Advanced serves as a resource for other team members and is active in the orientation of new staff. May be required to make home visits.
  • Provides direct and indirect nursing care individualized to the patient's needs.
  • Collects and records relevant patient health data accurately, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation and co-worker feedback.
  • Evaluates health data to identify patient problems, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
  • Implements and documents specified interventions with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation
  • Identifies deviations from expected patient outcomes, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation.
  • Adheres to approved, research-based practice standards, with no more than 2-3 exceptions, as measured by direct supervisory observation and random supervisory chart review.
  • Directs and supervises patient care provided by other LPNs (LPN Ambulatory Care Advanced only) and support personnel according to department guidelines, as measured by co-worker feedback and managerial observation.
  • (LPN Ambulatory Care Advanced) Identifies practice problems and consistently seeks solutions through appropriate communication channels
  • Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures, with no more than one exception per year, as observed by supervisor and POC staff.
  • Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures with no more than one exception per year, as observed by supervisor and POC staff.
  • Advocates for patients to ensure that care preserves and protects the patient's autonomy, dignity and rights.
  • Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor.
  • Assures that patients have informed consent signed for indicated procedures as measured by random supervisory chart review.
  • Maintains current licensure, certification, and competencies.
  • Provides proof of current BLS validation to supervisor.
  • Completes required unit credentialing according to departmental policy by providing documented evidence of completion to supervisor.
  • Provides to supervisor evidence of completion all required annual training and competency validation
  • Practices within the New York State-defined LPN scope of practice 100% of the time, as determined by co-worker feedback and direct supervisory observation.
  • Acquires and maintains current knowledge and skills in his/her area of practice.
  • Participates in required number of hours educational activity related to job function (LPN Ambulatory Care: 4-8; LPN Ambulatory care Advanced: 8-12) annually and provides documented evidence of attendance or activity to supervisor.
  • (LPN Ambulatory Care Advanced) Demonstrates operational problem-solving ability through written exemplar, patient/family feedback, co-worker report or direct managerial observation.
  • Performs similar or related duties as requested or directed
  • Performs other duties as requested and observed by supervisor or manager

What we need from you
Education:
  • Graduation from a Practical Nursing program, required

Experience:
  • Graduate Nurse, required
  • Previous healthcare experience, preferred

License/ Certification:
  • Current NYS licensure as a Practical Nurse or valid interim permit, required
  • BLS certification within 6 months of hire, required
  • Additional credentials are determined by specific work unit, required

We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. At the discretion of leadership, this program may terminate or be suspended at any time.
Bonus Amount:
  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
  • LPN sign on bonuses are awarded in the amount of $10,000 for full time

Pay Transparency
Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.
Pay Range Minimum:
USD $24.11/Hr.
Pay Range Maximum:
USD $34.84/Hr.
