Job Summary
We are seeking a compassionate and dedicated Registered Nurse for our Home Health department. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong background in various nursing specialties, excellent patient assessment skills, and the ability to work independently while collaborating with a multidisciplinary team.

Duties

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications and treatments as prescribed, including IV infusion and injections.
  • Monitor patient vital signs and report any significant changes to the healthcare team.
  • Provide education to patients and their families regarding health management and care plans.
  • Assist with activities of daily living, including tube feeding and catheter care.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
  • Implement infection control measures and ensure a safe environment for patients.
  • Support patients with developmental disabilities, Alzheimer’s care, dementia care, and other specialized needs.
  • Participate in discharge planning and case management to facilitate smooth transitions of care.

Requirements

  • Current Registered Nurse (RN) license in the state of practice.
  • Experience in home health or community-based nursing preferred.
  • Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth.
  • Strong clinical skills in areas such as critical care, geriatrics, pediatrics, acute care, or hospice care.
  • Familiarity with ICD-9/ICD-10 coding and medical terminology is advantageous.
  • Ability to perform phlebotomy, venipuncture, catheterization, and other nursing procedures.
  • Knowledge of pain management techniques and acute pain management practices.
  • Experience working in various settings including hospitals (Level I/II trauma centers), nursing homes, assisted living facilities, or outpatient clinics is preferred.
  • Excellent communication skills and the ability to work effectively with diverse populations.
