Registered Nurse (RN) - Hospice
Position Summary
The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.
Responsibilities
  • Perform initial and ongoing patient assessments
  • Develop, implement, and update individualized plans of care
  • Coordinate care with the IDT, physicians, patients, and families
  • Provide patient and caregiver education regarding disease process, medications, end-of-life care, and safety
  • Determine visit frequency based on patient needs and acuity
  • Supervise Hospice Aides and LVN/LPN staff
  • Document clinical findings, interventions, and patient progress accurately
  • Report changes in patient condition to physicians and team members
  • Practice infection control and standard precautions
  • Participate in QAPI and hospice in-service education programs
  • Provide on-call services as assigned
  • Promote hospice philosophy and quality patient care
Qualifications
  • Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing
  • Current RN license in the state of practice and/or compact license eligibility
  • Must have hospice experience
  • Understanding of hospice regulatory requirements, documentation standards, and compliance guidelines
  • Strong clinical, communication, and interpersonal skills
  • Knowledge of end-of-life and palliative care
  • Ability to read, write, and communicate effectively in English
