Silver Care is seeking a compassionate, dependable, and experienced Part-Time Home Care Nurse to join our team!
This position is ideal for a nurse looking for a flexible part-time opportunity providing in-home assessments, supervisory visits, care planning, and caregiver support throughout the Hampton Roads area.
Position: Part-Time Home Care Nurse
Location: Hampton Roads, Virginia
Norfolk • Virginia Beach • Chesapeake • Portsmouth • Suffolk • Hampton • Newport News • Yorktown • Williamsburg • Smithfield • Windsor and surrounding areas
Responsibilities
  • Conduct initial client assessments and ongoing supervisory visits
  • Assess client needs and develop or update individualized care plans
  • Complete required client documentation and paperwork accurately
  • Communicate with Silver Care staff and community partners regarding client needs and service authorizations
  • Monitor client progress and changes in care needs
  • Provide guidance and training to Personal Care Aides based on client care plans
  • Observe and verify Personal Care Aide skills and competencies as required
  • Assist with personal care needs when necessary
  • Travel to clients' homes throughout the designated service area
Qualifications
  • Valid nursing license in the Commonwealth of Virginia

  • Home care or similar healthcare experience preferred
  • Strong assessment, communication, and documentation skills
  • Compassionate and professional approach to client care
  • Ability to work independently and manage a flexible schedule
  • Comfortable utilizing electronic documentation systems such as AxisCare
  • Valid driver's license and reliable transportation
  • Ability and willingness to travel to client homes throughout the service area
  • Comfortable visiting homes that may have pets or smoking
