  • Providing skilled nursing care under the direction of an RN and physician

  • Following the established Plan of Care and physician orders during each patient visit

  • Monitoring patient conditions and reporting significant changes to the RN Case Manager and care team

  • Educating patients and caregivers on disease management, medications, and self-care techniques

  • Supporting patient safety through individualized care planning and risk prevention strategies

What You'll Bring

Graduate of an accredited Associate Degree Nursing program

Current state LVN/LPN license in good standing

Minimum of one (1) year of clinical nursing experience

Home Health or Hospice experience preferred

Strong clinical assessment and communication skills

Ability to lift, push, pull, and support up to 50 pounds as needed

Valid driver's license, reliable transportation, and current auto insurance

Willingness to travel locally (approximately 50%)
