Home Health PRN Registered Nurse (RN)

Your Responsibilities as a PRN RN:
As a Home Health Registered Nurse, you will manage a caseload of patients and provide comprehensive nursing care throughout their healthcare journey.
Responsibilities include:
* Completing skilled nursing assessments and developing individualized plans of care
* Performing admissions, routine visits, recertifications, and discharge visits
* Evaluating patients' physical, psychosocial, environmental, and educational needs
* Coordinating care with physicians, caregivers, and interdisciplinary team members
* Monitoring patient progress and identifying changes in condition
* Updating and revising care plans to ensure optimal outcomes
* Educating patients and families on disease management, medications, and self-care techniques
* Maintaining accurate and timely clinical documentation
* Helping prevent avoidable hospitalizations through proactive intervention and patient education
* Delivering compassionate, patient-centered care while promoting safety and independence
Qualifications:
* Current, unrestricted Massachusetts Registered Nurse (RN) license
* Minimum of one (1) year of nursing experience in a clinical care setting
* Ability to commit to at least one full day (8 hours) per week
* Valid driver's license, reliable transportation, and current auto insurance
* Willingness to travel throughout the assigned territory
* Previous Home Health experience strongly preferred
* Hospice experience preferred
* Experience with HomeCare HomeBase (HCHB) preferred
* Associate or Bachelor's Degree in Nursing
* Physical Requirements -Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
