Part time, 2 shifts per week 8am-4pm.  

A Grief Services Clinical Facilitator facilitates various key components and processes of the WSC Bereavement Services in tandem with the Manager. Responsible for client outreach and triage, coordination of service activity, and provision of support to programmatic efforts as well as counselor functions. Participates in support group facilitation and maintains small client caseload.

Essential Duties and Responsibilities for a Grief Services Clinical Facilitator:

Responsible for incoming outreach.

Facilitate triage of Bereavement Services and Community Grief Support.

Provide technical assistance to facilitate virtual telehealth activity.

Manage client appointments, missed visits, and coordinate consents.

Provide enhanced telephonic support to clients for enriched outreach and engagement.

Responsible for monthly outreach calls.

Coordinate correspondence program.

Prepare and maintain courtesy files.

Manage and maintain grief resource database and materials.

Manage group registration, scheduling, and virtual tech practice.

Coordinate and/or assist with various internal/external event programming in conjunction with team members, administration, and volunteers.

Support WSC/Bereavement Manager and Grief Counselors daily functions/operations.

Provides administrative back up support for team overall.

Assist with supervising intern tasks and assignments.

Facilitate Support Group(s).

Maintain customized small counseling caseload.

Qualifications for a Grief Services Clinical Facilitator:

Education/Certification

Masters' Degree in Social Work/Counseling

NYS Licensure required (LMSW, LCSW, LMHC)

Must have a current driver's license, free of serious violations, and vehicle required to travel offsite.

Experience

At least 5 years of experience counseling in healthcare or mental health setting.

Experience and expertise in grief & loss with a strong knowledge base of community resources and healthcare organization
