 Provider Support Specialist Aging & Community-Based Services


Location: Leominster, MA  

Schedule:  Part-time role with hours between 8 AM and 5 PM, M-F.

Work Environment: Onsite  

Compensation: $19-$21

What You'll Do: As a Provider Support Specialist, no two days are exactly the same. You may:


? Speak with consumers by phone to ensure their services have started successfully and are meeting their needs.

? Help resolve concerns related to home care, transportation, meals, and other community-based services.

? Work closely with service providers, case managers, caregivers, and agency staff to coordinate services and solve problems.

? Follow up on service requests to help ensure consumers receive timely support.

? Research and resolve service-related questions and concerns.

? Maintain accurate consumer records and service information.

? Monitor service implementation and identify opportunities to improve the consumer experience.

? Build positive relationships with consumers, providers, caregivers, and community partners.

? Participate in team meetings, trainings, and professional development opportunities.

What We're Looking for Required Qualifications

  • High School Diploma or GED.
  • Strong communication and customer service skills.

  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with Microsoft Outlook, Word, Excel, Teams, and other computer applications.
  • Ability to learn agency databases and documentation systems.

Preferred Qualifications

  • Associate degree in Human Services or a related field.
  • Experience in human services, healthcare, customer service, care coordination, or aging services.
  • Experience working with older adults, individuals with disabilities, or family caregivers.
  • Experience coordinating services, solving problems, and working with multiple stakeholders.
