Become the Senior Case Manager you always wanted to be

  • Performs case management duties and tasks at an advanced level.
  • Demonstrates understanding of patient outcomes, hospital goals and the department's impact on both.
  • Analyzes LOS, costs, outliers, and potential discharge planning issues.
  • Assigns patient caseload to department members and self for optimal delivery of Case Management services.
  • Acts as an interdisciplinary team leader and collaborates with other departments, medical staff, and hospital leadership.
  • Acts as a preceptor for new case management staff or students - assists with onboarding, orientation, training.
  • Maintains knowledge and understanding of hospital performance improvement programs, CMS Conditions of Participation, Medicare Coverage Criteria, and relevant Joint Commission standards.
  • Oversees department staff and operations in the absence of the DCM.

Qualifications

  • License or Certification:
    • CCM or ACM certification required.
    • For RN: Must hold a valid, unencumbered license in state where the hospital is located or from an appropriate Nursing Licensure Compact state.
    • For licensed or certified health care professional other than RN: If licensure is available within the state where the hospital is located and is applicable to the individual's educational level (e.g., BSW), the individual must hold a valid, active, unencumbered license.
  • Minimum Qualifications:
    • For RN: Minimum of associate degree in nursing (AND or ASN) required. Bachelor's degree (BSN) preferred. A diploma in nursing is acceptable only in those states whose minimum requirement for RN licensure is a diploma rather than an associate degree.
    • For Licensed or certified health care professional other than RN: Minimum of bachelor's degree in a health or human services-related field. Master's degree preferred.
    • Minimum 2 years of experience in Case Management, including Utilization Review and Discharge Planning in an inpatient rehabilitation setting.
